The project is located on Dull Road (SR 1170) over Ellison Creek (Class C) in Forsyth County. I have attached a pre- construction notification application, a storm�vater management plan and permit plans. The project will permanently impact 50 linear feet (Y-culvert, 4Y-bank stabilization) and temporarily impact 48 liuear feet (dewatering) of the Ellison Creek. The culvert site will be dewatered during construction in accordance with NCDOT's Best Management Practices. This permit application is a courtesy copy for the USACE for a non-reporting NW3, so no written concurrence is requested. The recommended (Description of Minimization of Water replacement for these structures is a single Corrugated Aluminum Structural Plate Pipe Arch 17'-0" x 10'-10" with an aluminum headwall and endwall. The existing channel Quality Impacts) varies in width from 6' to 15' through the project site. The recommended culvert will be buried 1' below the natural stream bed and will be fitted with sills at the inlet and outlet, and will also have one baffle placed in the middle of the culvert. The sills and baffle will be 2' high and will be notched to provide a 10' low flow channel width. A floodplain bench will be created at the inlet and outlet of the recommended structure to accommodate high flow conveyance while providing a normal channel width during low flow conditions. The banks of the floodplain bench will be laid back at 1.5:1 and lined with class II rip rap, while the bench will be established using coir fiber matting. The existing drainage patterns will be maintained through the existing ditches. PSRM will be used as toe protection through the ditch in the NE quadrant of the project site.
