1. Level Spreader #2 location has been moved to a slope that is 3.7% as to not exceed the
allowable 8%. We have provided the revised plans and calculations to further explain the
location and support the revised location. We have provided the checklist, level spreader
worksheets, bypass box calculations, drainage area maps, photographs, and sign O&M for
each level spreader

Dear Sir or Madam:
On March 26, 2008, the Division of Water Quality (DWQ) received your application dated
March 24, 2008, to fill or otherwise impact 146 linear feet of perennial stream, and 8,465 square
feet of Zone 1 Neuse River basin protected riparian buffers and 4,484 square feet of Zone 2
Neuse River basin protected riparian buffers to construct a residential development at the site.
On July 31, 2008, and August 1, 2008, the DWQ received additional information from you,
however, the DWQ has determined that your application remains incomplete and/or provided
inaccurate information as discussed below. The DWQ will require additional information in
order to process your application to impact protected wetlands and/or streams on the subject
property. Therefore, unless we receive five copies of the additional information requested
below, we will place this project on hold as incomplete until we receive this additional
information. If we do not receive the requested information, your project will be formally
returned as incomplete. Please provide the following information so that we may continue to
review your project. This Request For More Information replaces the Request For More
Information dated September 2, 2008.
Additional Information Requested:
1. The slope in the filter strip of the proposed level spreader number 2 exceeds the
allowable eight percent. Please either relocate this level spreaders to a location where the
filter strip slope is appropriate for a level spreader or provide a BM that removes a
minimum of 85 percent TSS and 30 percent TN from the stormwater generated by
drainage area 2. For the proposed BMP, please provide a completed BMP Supplement
Form, with all the required items. The BMP Supplement Forms are available at:
http://h2o.enr.state.nc.us/suibmp forms.htm. Please include both the Design Summary
and the Required Items Checklist
along with all the required items listed therein.

Operation and Maintenance Agreement
