Reference the subject permit request, for which I have enclosed a copy of the original transmittal
letter dated September 3, 2008.
Please find enclosed a revised drawing of crossing number 306. The revised drawing depicts a
1055 foot directional drill in lieu of the 600 foot originally submitted. Please insert this drawing in place
of the drawing submitted in my September 3, 2008 correspondence.
Please advise if you have any questions regarding this change.

In reference to the subject project, please find enclosed the following:
One (1) set of the following plan excerpts:
A. Index map Sheet No. 2 and creek crossing plan drawings from Plans (F-1303 Blue)
for the subject contract.
B. Standard Drawing SD 70010 which shows typical erosion and sedimentation control
for ditch, stream, creek and river crossings installed by open cut.
2. Seven (7) copies of the signed permit forms.
3. Copy of letter from DWQ regional office removing buffers from some crossings.
4. Copy of stream crossing summary sheet.
5. Copy of letter from State Clearing House.
6. Copy of letter to US Army Corps of Engineers.

This project consists of constructing 5.4 miles of 12" potable water transmission mains from Langs
Cross Roads into Farmville. Crossings installed by horizontal directional drilling will have six (6) feet
minimum bury depth. Ditch crossing installed by open cut will have four (4) feet minimum bury depth.
We are not aware of any wetlands that will be open cut by this project.
Please find enclosed MAI Check No. 023943 in the amount of $240.00 for 401 permit. Request
permit or permit waiver at your earliest convenience.
