REQUEST FOR MORE INFORMATION
Dear Sir or Madam:
On March 12, 2008, the Division of Water Quality (DWQ) received your application dated
March 12, 2008, to fill or otherwise impact 0.375 acres of 404/wetland and 149 linear feet of
perennial stream to construct the proposed residential subdivision at the site. On August 25,
2008, the DWQ received additional information from you, however, the DWQ has determined
that your application remains incomplete and/or provided inaccurate information as discussed
below. The DWQ will require additional information in order to process your application to
impact protected wetlands and/or streams on the subject property. Therefore, unless we receive
five copies of the additional information requested below, we will have to move toward denial of
your application as required by 15A NCAC 2H .0506 and will place this project on hold as
incomplete until we receive this additional information. Please provide the following
information so that we may continue to review your project.
Additional Information Requested:
All stormwater drainage from the entire site must be directed to one of the retention
basins.
2. Please provide properly designed diffuse flow measures for the drawdown from the 1"
storm along with the supplement forms and required items checklists. Storms larger than
the design storm must be conveyed to another location where flow is non-erosively
conveyed to the wetlands with appropriate dissipation devices. Please provide details of
the dissipation devices.
3. Please determine the location of the SHWT and insure that the invert of the retention
basin is at least one foot above the SHWT so that the adjacent wetlands will not be
drained.

4. Please provide the following items to aid in DWQ's review and proper installation of the
retention devices:
a. Plans (1" - 50' or larger) of the entire site showing delineated drainage basins
(include proposed drainage easement and overflow devices).
b. Plan details (1" = 30' or larger) for the retention devices showing basin
dimensions, outlet structures, overflow devices and vegetation specifications.
c. Section view of the retention basins (1" = 20' or larger) showing side slopes of 3:1
or lower.
d. A construction sequence that shows how the retention basins will be cleaned out
and converted to permanent devices after the entire drainage area is stabilized.
e. A copy of the signed and notarized operation and maintenance (O&M) agreement.
f. A soils report showing the elevation of the SHWT at the proposed locations of the
retention devices that is based upon an actual field investigation and soil borings.
Please submit this information within 30 calendar days of the date of this letter. If we do not
receive this requested information within 30 calendar days of the date of this letter, your project
will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian
