A check for the application review fee is also enclosed. This work is to remove and replace an existing Boathouse with a new larger Boathouse structure. This work will require the new Boathouse foundation to extend 12 -FT into the waters of Lake Toxaway in order to maintain the required 50 -Ft setback from the existing on-site wastewater system. The 12 - Ft foundation extension will be a permanent fill. This will require special approvals and a variance from LTCA and the POA. The work will be performed in -the -dry using a coffer dam to create a dry work area. Lake Toxaway has a water quality classification of B; Tr. All disturbed areas not to be filled or flooded will be vegetated pursuant to the seeding schedule in the Erosion and Sediment Control Planning and Design Manual. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: This home site located in Toxaway Estates, a private residential development in western Transylvania County, NC. The lot borders Lake Toxaway which has a classification of B: Trout. The general land use is residential. There is an existing 14'x18' Boathouse located on site, which is to be removed and replaced with a new 28'x30' Boathouse. 3b. Explain the purpose of the proposed project: The existing boathouse foundation is failing and the structure is unsafe to occupy. Describe the overall project in detail, including the type of equipment to be used: NEW WORK - REMOVE & REPLACE EXISTING BOATHOUSE - LOCAL VARIANCE REQUIRED; This Project is to remove the existing 14'x18' Boathouse and replace with a new 28'x30' Boathouse as described herein. The Foundation for the Boatslip portion of the new Boathouse shall Toxaway Block Precast Concrete Segmental Blocks with out -to -out dimensions of 14'x30'. The Foundation for the Kayak Storage portion of the new Boathouse shall be a cast -in-place concrete retaining wall. The work will also include a 12'x14' new wooden Dock. The Architectural features of the new Boathouse shall comply with the 2016 edition of the Lake Toxaway Community Association Architectural Review Standards. The existing property lines and the Side Yard Setback shall be located, marked and flagged by a Professional Land Surveyor prior to beginning work. Verify actual setback requirement with the most current Lake Toxaway Community Association Architectural Review Standards. Perform incidental excavation as necessary to install the new Boathouse Foundation. Work to be performed using hand labor & small equipment. All work shall be performed "IN -THE -DRY" . This will be accomplished by installing a temporary coffer dam around the proposed work area, or by scheduling the work during a lake draw down. Where a coffer dam is used, the work area will be dewatered by pumping. All dewatering discharge shall have a turbidity of not more than 10 NTUs above background or as otherwise specified in DWQ's Turbidity Standards (15A NCAC 02B.0211(3.)(k.)). To satisfy the required turbidity limit, the dewatering pump discharge shall be filtered through a Bag Filtration Unit equal to Model BF -400 from Rain -for - Rent with a 100 to 1 micron nominal rating. The turbidity level of all discharge shall be monitored with calibrated turbidity meters and the filtration equipment shall be adjusted as necessary to insure compliance with Water Quality Standards. The water around the work areas shall likewise be monitored with calibrated turbidity meters and the turbidity curtains shall be installed in addition to the coffer dam as needed to insure compliance with Water Quality Standards. Uncured concrete shall not be allowed to come into direct contact with surface waters. This will be accomplished by establishing a "dry work area" as described above. "High Early" cementious material shall be in the stone work to minimize curing time. DOCK PILINGS INSTALLED IN WATER - Pilings may be placed by auger, jetting or drop hammer. Pilings shall receive a final set by drop hammer or other approved method, acceptable to the Building Inspector to ensure compaction of material at end bearing. The tip -elevation for all pilings shall be field determined during installation. All pilings shall bear on satisfactory insitu material having a minimum bearing capacity of 3000 PSF. The minimum embedment depth shall not be less than 6 -Feet. The actual embedment depth shall be as required to insure pilings bear on satisfactory insitu material having a minimum bearing capacity of 3000 PSF. All building materials shall be suitable of exterior use. All Pilings for the Boat Dock that are to be located in -water shall be pressure treated for fresh water immersion. PERMANENT CONCRETE FILL PLACED IN LAKE TOXAWAY - Due to the close proximity for the existing on-site wasterwater system, a portion of the Boathouse Foundation will extend 12 -FT into the waters of Lake Toxaway. This is necessary to maintain 50 -FT minimum setback from surface water (wet slip) to the existing on-site wastewater system. This will result in 196 CF (7.5 CY) of concrete foundation extending beyond the shoreline of Lake Loxaway. The total Boathouse Foundation below the normal pool elevation of Lake Toxaway will be 725 CF (27 CY). VARIANCE, SPECIAL AUTHORIZATION and 401 / 404 PERMITTING ARE REQUIRED. The proposed new Boathouse is to extend 12 -FT into Lake Toxaway and will be located within the side yard setback and immediately adjacent to the side property line. This is necessary to maintain a 50 -FT setback from the existing on-site wastewater system. Prior to beginning construction or ordering materials, the Owner and/or the Contractor shall obtain a Variance and Special Approval from the Lake Toxaway Community Association and from the POA for the Boathouse to extend beyond the shoreline into Lake Toxaway, and for the Boathouse to be located within the side yard setback. A Written Authorization - for -Construction and a Release -of -Claims shall also be obtained from the adjacent property Owners prior to beginning work. The Authorization -for -Construction and the Release -of -Claims shall be prepared by an NC licensed Attorney experieneced in environmental and real estate law. No work shall begin without these written Authorizations and Release -of -Claims. A 401 Water Quality Permit from the NCDEQ-DWR and 404 US Army Corps of Engineers Permit are also required. The new Boathouse Foundation shall be Toxaway Block Precast Concrete Segmental Blocks, with out—to—out dimensions of 30'x30'. New Boathouse shall be complete with one (1) 12,000 Lb Boat Lift, kayak storage, and dock. The Architectural features of the new Boathouse shall be as described on Sheet A-1 of the Plans prepared by C. Allen Brown, Architect. Comply with the latest edition of the Lake Toxaway Community Association Architectural Review Standards The existing property lines and the Side Yard Setback shall be located, marked and flagged by a Professional Land Surveyor prior to beginning work. Verify actual setback requirement with the most current Lake Toxaway Community Association Architectural Review Standards. 2.0 As a result of the site investigation, a number of observations have been made and recommendations provided. It should be noted that the observations and conclusions stated herein are based on information available as of this writing. It is conceivable that additional information may be forthcoming which bears on these observations, conclusions and the overall design. The right is reserved, therefore, to review and modify all observations, conclusions, and design features at any future point in time should, in fact, additional information become available. The comments, recommendations, and design features provided herein are applicable only to the new Boathouse. 3.0 All work shall be in accordance with North Carolina State Residential Building Code, OSHA Safety and Health Requirements, US Army Corps of Engineers 404 permit requirements, NCDENR Division of Water Quality 401 permit requirements, NC Sedimentation Pollution Control Act, and all other applicable codes, ordinances, and regulations. The Contractor shall coordinate all work with the Lake Toxaway Operations Manager, 16801 Rosman Hwy, Lake Toxaway, NC 28747. 4.0 PERMITS. The Owner and/or Contractor shall obtain all required permits, rights—of—entry & authorization Lake Toxaway Community Association prior to beginning work. Locate, flag and protect all underground utilities. Do not locate any portion of the work within and easement, set—back, or R/W. 5.0 The Contractor shall exercise care to preserve the natural landscape and waterways and shall conduct the work so as to prevent any unnecessary destruction, scaring, or defacing of the natural surroundings in the vicinity of the work. All unnecessary destruction, scarring, damage or defacing of the natural landscape shall be corrected at the contractor's expense. 6.0 All construction materials and debris shall be removed from the site upon completion of the project and the area shall be graded such that all surfaces drain naturally, blend with the natural terrain, and are left in a condition that facilitate sown ground covers and natural revegetation. 7.0 The Contractor's activities shall be performed by methods that will prevent entrance, or accidental spillage, of solid matter, contaminants, debris, or other pollutants and wastes into streams, flowing or dry watercourses, lakes or underground water sources. 8.0 Runoff and discharge shall be monitored to insure that no sediment is discharged to the downstream watercourse. 9.0 TURBIDITY CONTROL. The Contractor shall not violated water quality stream standards during the performance of this work. The Turbidity Level in the lake waters shall not exceed 10 NTUs at any time during performance of this work pursuant to 15A NCAC 02B.0211 (3)(k). The Contractor shall employ any and all methods necessary to prevent a violation of the Turbidity Standards. These measures include, but are not limited to; turbidity curtains around the work, haybails, silt fence, stone check dams, etc. Where turbidity curtain are necessary, curtains shall remain in place until the turbidity in the work areas return to original background levels. O Ld J C-1 SHEET I OF 13 GENERAL NOTES AND SPECIFICATIONS. 1.0 All work shall be in accordance with the latest edition of the North Carolina Building Code, ACI 301 — Specifications for Structural Concrete for Buildings, ACI 318 & 318R — Building Code Requirements for Structural Concrete, ACI 548.1R Guide for Use of Polymers in Concrete, ACI 546R Concrete Repair Guide, AISC Steel Construction Manual, OSHA Safety and Health requirements, and all other applicable codes, ordinances, and regulations. 2.0 Contractor shall field verify all dimensions and elevations prior to beginning work or ordering materials. Do not perform any work or disturbance within any set—back, easement, right—of—way, floodway or flood fringe. Contractor shall obtain all required permits prior to beginning work. 3.0 Contractor shall verify and establish all Finish Floor Elevations (FFE) prior to beginning work. Do not locate any part of the structure within any set—back, easement, right—of—way, floodway or flood fringe, NCDENR regulated set—back, trout stream buffer zone (or other buffer zone or set back). Contractor shall obtain all required permits prior to beginning work. Contractor shall locate all property boundaries prior to beginning work. Contractor shall locate, flag and protect all underground utilities prior to beginning work. 4.0 Design criteria and assumptions: Design Method = ASCE-7, Minimum Design Loads for Buildings and other Structures Soil Bearing Capacity = 2000 psf, assumed Floor Live Load = 40 psf Floor Dead Load = 15 psf 5.0 FOOTINGS. The bottom of footings shall be located below the frost line. Footings shall be stepped where it is necessary to change the elevation of the top surface of the footings or where the slope of the bottom surface of the footings will exceed 0.1 unit vertical in ten units horizontal (1% slope). All individual pier footings shall be restrained from sliding with anti—slide keys, helical piers, etc). 6.0 CONCRETE. Concrete shall have a minimum compressive strength of 4000 psi at 28 days and shall have 5 to 7% entrained air. 7.0 REINFORCING STEEL. All reinforcing steel and anchor dowels shall conform to ASTM A 615, Grade 60 and shall have standard deformations. All footer reinforcing bars shall have a minimum of 3" of concrete cover. All rebar lap splices shall be 60 bar diameters unless otherwise noted. All footer steel shall be bonded and grounded per the NEC. 6.0 WOOD FRAMING. All wood framing materials shall be #1 Southern Yellow Pine or better and shall be pressure treated. All floor joists shall be attached to the girders and beams with galvanized joist hangers.Provide triple (3) floor joists beneath all parallel partition walls and beneath all bathroom tubs. 8.0 STRUCTURAL STEEL: All steel shapes shall conform to ASTM A-992-50. All steel plates & rods shall conform to ASTM A36. All steel members except bolts shall receive one shop coat of inhibitive primer and two field coats of paint, color specified by Owner. 9.0 BOLTS: High strength bolts and their installation shall conform to ASTM Specification A-325. Tightening of these bolts is recommended by the turn—of—the—nut method per AISC Specifications for Structural Joints. 10.0 MULTI—PLY WOOD BEAMS. All individual plys of multi—ply beams must be fully adhered to insure uniform transfer of load. Connect plys together with 5/8" dia gals bolts at 12" o.c. staggered pattern. All work shall be in accordance with the 2012 edition of the North Carolina State Residential Building Code, ACI 301 — Specifications for Structural Concrete for Buildings, ACI 318 — Building Code Requirements for Structural Concrete, AC1 306 — Cold Weather Concreting and all other applicable codes, ordinances, and regulations. 2. EXCAVATION, FILL AND BACKFILL FOR Retaining Walls — Only satisfactory material shall be used for fill or backfill behind retaining wall. All trip tickets shall be maintained by the Contractor as part of the project record. NO WATER SHALL BE ADDED TO THE FRESH CONCRETE ON SITE. 4.1 COLD WEATHER CONCRETE REQUIREMENTS — See ACI 306. 4.1.1 Adequate equipment shall be provided for heating concrete materials and protecting concrete during freezing or near—freezing weather. 4.1.2 All concrete materials and all reinforcement, forms, fillers, and ground with which the concrete is to come into contact shall be free from frost. 4.1.3 Frozen materials or materials containing ice shall not be used. 4.1.4 The ambient temperature shall be 40 degrees F and rising before concrete beginning concrete placement. 5. REINFORCING STEEL 5.1 Reinforcing steel shall conform to ASTM A615, grade 60 (fy=60,000 psi) and shall have standard deformations. All lap splices shall be 60 bar diameters unless otherwise noted. 5.2 All reinforcing steel shall be accurately and securely tied and anchored in place to prevent dislocation during the placement operation. 5.3 Provide corner bars, 48 bar diameters x 48 bar diameters, at each continuous footing and foundation wall corner. 5.4 Provide 2" clearance for all wall steel. Do not disturb the underground stormwater retention system or foundation drainage & waterproofing system located adjacent to the proposed retaining wall. 11.0 CONDITIONS FOR USE OF THIS PLAN — All portions of the existing structure were designed by others. Prior to beginning construction or ordering materials, the Owner and/or the Contractor shall obtain a Variance and Special Approval from the Lake Toxaway Community Association and from the POA for the Boathouse to extend beyond the shoreline into Lake Toxaway, and for the Boathouse to be located within the side yard setback. A Written Authorization—for—Construction and a Release—of—Claims shall also be obtained from the adjacent property Owners prior to beginning work. The Authorization—for—Construction and the Release—of—Claims shall be prepared by an NC licensed Attorney experieneced in environmental and real estate law. A 401 Water Quality Permit from the NCDEQ—DWR and 404 US Army Corps of Engineers Permit are also required. No portion of the new structure shall extend beyond any adjacent property line.
