Joel —This is a follow-up to the voice message I left for you this morning. NCDEQ received the application package for
the subject project on May 5, 2017, however the application cannot be accepted as submitted. Please provide the
following information by May 12, 2017, or the application will be returned as incomplete.
1. The applicant, Childress Klein Properties, on SWU-101 is not consistent with the entity on the deed, CKAfton
Ridge Land, LLC. Which entity is the applicant that we should use for this permit application? If the applicant is
different than the property owner, then 111.2.a. on SWU-101 must be completed.
2. Please provide 2 full-size hardcopies of the site plan and grading plan sheets. These were not included in the
submittal. Without those plans, we cannot perform an adequate first review.
3. Please provide a completed supplement form for the SCM. The latest supplement form is called Supplement -EZ,
and can be downloaded from our website. (Also note, the previous O&M Agreement forms are now replaced by
