Enclosed is a copy of the Compliance Evaluation Inspection Report for the inspection conducted at the
subject facility on May 5, 2017, by Edward Watson and. Ori Tuvia. The MRO would like to thank Mr.
Teg Williams and Mr. Scott Smida for their assistance during the site visit.

The recovery well next to the treatment system was reviewed during the inspection. The well was in
good order. The MW -23A broken hinged was reported to be repaired. However, the wells were not
reviewed during the site visit._
It is expected that the system will be shut down to observe rebound of the treated area. The GW
monitoring wells would be sampled to observe rebound and therefore indicate if remediation has been
completed to an acceptable level. If it has been determined that system be turned off is warranted, the
permittee will request permit rescission.
The rebound study will be for the period of one year. The MRO has requested that we be kept informe,
of any decisions made that would affect the permitted treatment system, so that we are made aware if
the. permit will a rescinded, or if the system will be turned back on for futher remediation. The MRO is
also aware the system start-up may affect Chronic Toxicity testing negatively during system start up
due to chemical build-up that could occur during the rebound study period.

Comment: The permit was present and available to be reviewed during the site visit. Teg
Williams of
CH2MHi11 commented that they were considering
shutting down the system to monitor site
rebound as part of a possible permit rescission.
It was requested by MRO DWR that we be kept in communication with the DWM regarding
correspondence for system closure so we are made aware that the permit was able to be
rescinded.
