Hello James, Ian, & Amy; hope you are all doing well and thanks for all your help. James, thanks for
speaking with me Friday. I've spoken with the grace haven church staff and attached are the 2 items
that will be submitted to your offices tomorrow. The plan sheet was submitted with the original
packet. We are planning on keeping the existing east crossing for future construction activities.
Though there aren't any details, they eventually want to build a church out there on the large upland
area to the west of the (2nd) west stream. To prevent the large trucks from tearing up the proposed
new road, they would like to keep the existing culvert near Cedar Creek for the construction vehicles.
Though I will be out of town, y'all and friends/coworkers are invited to the 5K that the church is having
on November 1St. The finish is on this property near the pavilion/shelter and it's a beautiful race
route. Attached is the brochure.

Hello Amy, hope you had a great weekend with the family. We'll be submitting the 401 package to
your office this Thursday for Grace Haven church. It will be a NWP 14(GC 3704) per James' email
below. Diffuse flow to both tributaries of Cedar Creek will be provided. Regarding stormwater, it is
very minimal (<5%) but we'll provide a breakdown on the PCN.

Jay,
I am handling this project for Eric. After discussions with Eric Alsmeyer, NWP 29 and 39 can't be used becuase you are
located in the FEMA floodplain. I think NWP 14 would be ok to use. You would not have to notify as long as you stay under
150 feet and you aren't impacting special aquatic sites (i.e wetlands). If you are going to impacts wetlands or other special
aquatic sites then a PCN would be required. Also i think DWQ requires notification if a NWP 14 is being used. As for the
second stream crossing I would consider that a maintainance activity that would not require notification. I hope this has

Morning Jay.
You need to discuss this with the Corps first and then talk to us. If this does require a NW 39 or 29 with the
Corps, you will need to have written concurrence from DWQ since the impacts are in a buffered basin. Also,
there is a definite possibility you would be required to do stormwater.

Hope you are doing well. Amy, congratulations! I have a church site in near Franklinton that borders Cedar Creek. They
have an existing roofed pavilion that was built -1 year ago and the old farm road provides access to it and the remaining
western property. They're hoping to construct a new 18'W road farther away from Cedar Creek since it floods occasionally.
It would involve placing a new culvert in the 1St perennial stream and replacing the culvert on the 2nd perennial stream. We

Secondly, does the first perennial stream require a 401/404 permit since impacts will be less than 40'? Both streams are
-6' wide. I want to confirm my interpretation with you before I advise my client. If it does need a permit, would it be a NW P

Dear Mr. Lastinger & Ms. Karoly:
Thank for your assistance and DWQ staff assistance with this project. We are submitting the
following information for Grace Haven Baptist Assembly regarding our phone conversation
this past Friday (September 9/12/08) with Mr. Lastinger. We are requesting for Corps of
Engineers (COE) and NC Division of Water Quality (DWQ) written approval under Sections
404 and 401, Clean Water Act, 33 USC 1344 and the Tar-Pamlico River Buffer Rules.
The following information pertains to the existing first (east) stream crossing and the area to
the west of the existing second (west) stream crossing.
• Existing first (east) stream crossing - There is an existing culverted stream crossing on
the first (east) stream. This crossing is approximately 100-feet south of cedar creek.
Plans for this crossing in the immediate future are for it to remain. It will be used for
future construction traffic to prevent damage to the new road. Upon completion of the
future projects, it will be removed.
West area- There is a large upland area to the west of the second (west) existing stream
crossing. This area is currently being used as a recreation area and has a large network
of maintained trails. The proposed road end lines up with one of the existing trails.
Grace Haven currently provides worship and bible study services at Youngsville
Elementary School. Future plans are to construct a building in this large upland area
and provide these services on this site. Details of these plans are not known at this
time.
