After a site visit held April 27, 2017, the DWR has determined that your application is incomplete and cannot be processed. The application is on -hold until the following information is received: 1. Please update the impact table to clearly show culverts that were installed prior to the 2005 permit (old forest road culverts), culverts installed after the 2005 permit, and proposed culverts and/or existing culvert extensions for the current permit application. Make sure to include the number, type, and length of each culvert. Include information regarding whether each culvert was measured in the field or how the measurement was obtained. 2. Update the table to accurately reflect the type of culverts that exist on the property, specifically culvert # 7, which was incorrectly listed an as arch pipe. 3. For the culvert located between Lot no. 23 and Lot no. 40 that is proposed for removal, please provide details and plan sheets to illustrate how the culvert will be removed and stream channel restored. Please include the length of the culvert and update the impact table in the PCN. 4. The proposed project must comply with the stormwater management plan (SMP) condition of WQC 4092 (item 4 on page 3) (http://edocs.deq.nc.gov/WaterResources/DocView.aspx?dbid=0&id=493434&page=1&cr=1). Please explain how the project will meet this condition either through a state -approved local government SMP approval or through DWR. 5. Please provide a detail demonstrating how all proposed culverts and culvert extensions will comply with Conditions 9 and 11 (page 6) of WQC 4092. Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the proper consideration of the application. If all of the requested information is not received within 30 calendar days of receipt of this email, the DWR will be unable to approve the application and it will be returned. The return of this project will necessitate reapplication to the DWR for approval, including a complete application package and the appropriate fee.
