1 If buried, these pipe slopes wauid exceed 6°fo grade. As a result, we are not proposing to bury
these pipes since any fish passage wouid be very unlikely and head cuts would be prohable. A
riprap pad will be placed in the streambed at the �outlet of the pipe at station 16+0� to reduce
scour. This pipe wi111ikely remain perched.
2 There is an existing privafie wet pipe discharging into our ditch Iine at this lacation. We intend
ta cut a few feet off of that gipe, install a box and replace our pipe with a slightly longer pipe.
The net new permanent fill will be 2' for the box, 5' of additional pipe on the outlet end to
address the failed shoulder and 1Q' for the dissipater pad f+or a total of 17' at this station.
Dewatering adds another 20' af �emporary impact.

The replacement of the above noted wet pipes wilt requ�re temporary dewatering. An impervious
sandbag dike will be constructed upstream of the existing pipes to create a small pool. Clean
water will temporarily be pumped around the consfruction areas and will be discharged
downstream. Any dirty water located inside of the construction areas wili he pumped into a silt
bag if necessaty. Once the new pipes are in p�aee, the impervious dikes wiil be removed and
precanstruction st�reambed elevations will be restored.
The pipes at stations 16+00 and 23+$2 carry different UTs to Cobb Creek (class: C; Trout, +} in
the 7'Jew River Basin. This praject was reviewed far the presence of threatened and endangered
species in the iield and I have also included a query of the NCNHP database. Based on the
project area mapped with my request, the database search indicates that there are no records for
rare species, unportant natural communities, natural areas, or conservation/rnanaged areas within
the pz-oposed project boundary, or within a ane-mile radius of the project bauz�dary. There will
be approximately 1.9 acres of tree cutting at the project location due to cut and fill vvork. The
NLEB is listed as a protected sp�cies in this county but the project does not fall within 0.25 miles
of any red HUC identified in the SLOPES spelled out in the Nationwide Permit Regional
Conditions. We do not anticipate any irnpact to any other federally listed species.
Cultural resource compliance work has been completed. No archaealogical resources or historic
properties will be affec#ed by this project {see attachad}.
The project impacts are not significant in nature and should not remove or degade existing water
yuality uses. Stormwa#er wili be routed through grass or rock lined ditches, depending on the
ditch line slope. Dissipater pads wil] be installed out pipe outlets as appropriate. Pipe si�es used
will be the minimum necessary to create a safe roadr�vay. By utilizing the erosion control
measures outlined in the attachments to this application, dawnstream water quality sta�tzdards will
be protecied. �verall water quality in the area will be im�roved by paving the ABC travei lanes.
All appropriate BMFs will be used during construction.
Attached is a Preconstruction Notificatian Form, plan sheets with proposed erosion control
measures5 a marked county map and a marked topagraphy map.

Carolina Wildlife Resources Commission comment directly to ydu concerning the 404
Nationwide Permit application with a copy sent to me. Please note that a trout buffer variance
will be requested for land disturbance frvm stations 6+40 — 8+0� (lt.), 17+39 — 18-�47 (lt_) and
28+60 — 29-�22 {lt.).
