Subject: Stream determination request forms for MCOLF Oak Grove, and MCAS Cherry Point
(Ordnance)
Attachments: NCDWQ_StreamDeterminationRequest_MCOLF_OG_170503 pdf
Good Afternoon,
We are requesting stream determinations for two areas, MCOLF Oak Grove (1 origins, attached) and MCAS Cherry Point
(15 origins) and will be submitting stream determination request documents for each area Each document should
contain the following components
-Stream Determination Request form
-USGS and NRCS soil maps showing the stream origin/s
-Stream Identification form/s
-Stream photo log/s
The documents are fairly large, so I may need to send each component separately or use AMRDEC Please let me know
when you receive all components of each stream request packet
I also want to include a reminder that we are tentatively scheduled for stream origin verification on May 16 and 17tH,
2017 Jessica Guilianelli in the MCAS CP Environmental Office said you should not have any access issues as long as you
enter the base with me She said if they're operating under a higher level of security you may need to show your drivers
license and state employee ID badge We can discuss a meeting time/place as the day approaches Please don't
hesitate to let me know if any conflicts arise and we can adjust the schedule if need be
Thanks)
