Department of Transportation, 5501 Barbados Boulevard, Castle Hayne, NC 28429 Authorizing development in Brunswick County at the Atlantic Intracoastal Waterway, Bridee No. 198 on SR 1172 (Sunset Beach Bridge), as requested in the permittee's application letter dated 12/12/16, includine the attached worknlan drawin�31 dated 12/8/16, 12/12/16,and 12/15/16. This permit, issued on 1/11/17 , is subject to compliance with the application (where consistent with the permit), all applicable regulations, special conditions and notes set forth below. Any violation of these terms may be subject to fines, imprisonment or civil action; or may cause the permit to be null and void. 1) Unless specifically altered herein, this Minor Modification authorizes the placement of 1,901 linear feet of riprap along the causeway from Station 12 + 75 to Station 31 + 76, and the repair of erosion around the drop inlet near STA 31 + 00 +/- RT, all as depicted on the attached workplan drawings. NOTE: The work to repair minor erosion around the drop inlet neaz STA 31 + 00 +/- LT proposed in the permittee's letter dated 12/12/16 was previously approved by the N.C. Division of Coastal Management (DCM) in a Letter of Refinement dated 6/30/16. 2) The placement of riprap shall be limited to the areas as depicted on the attached workplan drawings. The riprap material shall be clean and free from loose dirt or any pollutant except in trace quantities. It shall be of a size sufficient to prevent its movement from the authorized alignment by wave or current action. The riprap material shall consist of clean rock or masonry materials such as but not limited to granite, marl, or broken concrete. (See attached sheet for Additional Conditions) This permit action may be appealed by the permittee or other qualified persons within twenty (20) days of the issuing date. This permit must be accessible on-site to Department personnel when the project is inspected for compliance. Any maintenance work or project modification not covered hereunder requires further Division approval. All work must cease when the permit expires on No expiration date, pursuant to GS 136-44.7B In issuing this permit, the State of North Carolina agrees that your project is consistent with the North Carolina Coastal Management Program. Department of Transportation Permit #22-07 Page 2 of 2 ADDITIONAL CONDITIONS 3) The activities authorized by this Minor Modification shall not result in any additional permanent or temporary impacts to wetlands or waters of the State, without permit modification. 4) If it is determined that additional permanent and/or temporary impacts are necessary that are not shown on the attached workplan drawings or described in the authorized permit application, a permit modification and/or additional authorization from DCM may be required. The permittee shall contact a representative of DCM prior to wmmencement of any such activity for this determination and any permit modification. 5) This Minor Modification does not eliminate the need to obtain any additional permits, approvals or authorizations that may be required. 6) This Minor Modification shall be attached to the original of Permit No. 22-07, which was issued on 2/27/07, as well as the amendment issued on 3/5/07, and all subsequent modifications and/or refinements, and copies of all documents shall be readily available on site when a Division representative inspects the project for compliance. 7) All conditions and stipulations of the active permit remain in force under this Minor Modification unless altered herein. NOTE: A minor modification application processing fee of $100 was received by DCM for this project.
