Multiple shut downs, crew quitting, etc. Brandon, I’ll need for you to address the items below. I have 500 other things going on. Let Dave know when things are addressed to your satisfaction. The project was shut down until crews could make necessary repairs to EC measures and remove sediment from jurisdictional areas. Much sediment remains in the jurisdictional stream downstream of the last permitted site (63+50). Please instruct crews to remove accumulated sediments in meanders bends and aggradational bars of the stream, leaving no more than ¼” skim layer of sediment. I estimate 3-7 five-gallon buckets worth of sediment remains in-channel from the SDO to approximately 100 - 150’ downstream. I observed several blobs of bearing grease throughout the project, of which could eventually enter surface waters. Grease is toxic to aquatic organisms and needs to be removed from the land surface and disposed of properly. Let me know when EC repairs and sediment removal is complete. I will conduct a follow-up inspection next week. Dave W. We are late notifying you and it’s my fault. Station 63+50 looks to be the worst of it. The rest appears minor. From: Greer, Brandon S Sent: Monday, April 03, 2017 3:53 PM To: Slaughter, Johnathan H <hslaughter@ncdot.gov> Cc: Eller, Douglas W <dweller@ncdot.gov>; Souther, Zachary S <zssouther@ncdot.gov> Subject: SR 1513A (Indian Grave) Heath, We experienced sediment loss on the project, due to the rains on 3/31 and 4/1. The project received 1.75-inches of rain. Cleanup and repair work began Saturday and continued Sunday and this morning, prior to today's rain event. The contractor was not able to reclaim all of the offsite sediment before today's rain. The contractor was able reinforce and install additional measures to assist in capturing the sediment within the project limits. Below is a summary of the locations. Sta. 34+00 Lt. and 36+00 Lt. - sediment loss was approximately five 5-gallon buckets. Sta. 42+11 - Sediment overtopped the 57 stone and a half bucket was outside of the project limits. Sta. 44+00-48+00 Lt. (fill section) - a portion of the filter stone was breached and roughly 2 buckets of material was removed. Sta. 63+50 Lt. - The outlet discharge on the newly installed pipe was undercut the rip rap along the foil slope. All areas were matted and covered. Approximately 7 CY of material left the project limits. Sta. 68+50 Rt. - approximately 1 CY of sediment was trapped in low area at the pipe inlet.
