continuing nature, not related to operation and/or maintenance problems, and you anticipate remedial
construction activities, then you may wish to consider applying for a Special Order by Consent (SOC).
If the above parameter was left off inadvertently, please send amended DMRs within 10 business days of receipt
of this letter to Raleigh at the address found on the front page of your DMR and also a copy to this office.
Your facility Will be required to report your Discharge Monitoring Reports electronically by December
21, 2016.
