This project meets the documentation requirements and approval procedures under NEPA, as defined by FHWA, for Low Impact Bridge Replacements. This project meets the standards of NCDOT's Minimum Criteria Rules. Date: Applicant Name: Applicant Address: March 13. 2017 North Carolina Department of Transportation P.O. Box 1584 Greenville NC 27835 Primary Contact for Project: Hon Yeunq Phone No.: 252-439-2800 Is this an after-the-fact application: Yes X No River Basin: Tar-Pamlico Stream Classification: C; Sw. NSW Regulatory Authorization Options for this Activity Federal: USACE Nationwide General Permit 3— Maintenance State: General Water Quality Certification #3687 and/or Buffer Authorization Local: None PfOjeCt DeSCI'Ipt10i7 — State project 17BP2.R.76 involves the replacement of Beaufort County Bridge #060004 on SR 1410 (VOA Rd.) over Aggie Run. The existing bridge consists of t@ 17' - 10", 4@ 17' - 0" and 1@ 17' - 7", reinforced concrete deck on timber joists. It is to be replaced with a 1@ 50' - 0", 1@ 50' - 0" and 1@ 35' - 0", 21" cored slab (135' total length, 33' total width) at the same location. In accordance with Tar-Pamlico River Buffer Regulations, the proposed bridge does not contain deck drains. All stormwater from the bridge and approaches will be collec[ed by a storm drainage system and discharged into the wetlands. The drainage system will outlet outside of the buffer limits in the southeast quadrant at -L- station 13+g6 RT with the outlet pipe set at 0.3% to reduce velocity into the wetlands. At northeast quadrant the fill slope was revised to steeper than 3:1 slope to eliminate fill in the existing ditch. The proposed ditch in the northwest quadrent maintains exist drainage pattem by utilizing variable width base ditch that tappers to 0' in depth and matches the width of existing drainage feature prior to reaching Aggie Run.
