at the subject facility on March 22, 2017, by Ori Tuvia and Brandy Costner. Michael Gantt's
cooperation during the site visit was much appreciated. Please advise the staff involved with
this NPDES Permit by forwarding a copy of the enclosed report.'
As was discussed during the inspection, DMRs must indicate all certified labs used for
the sampling done for that month's DMR.

Comment: The records reviewed during the _inspection were ortaanized and well maintained DMRs
COCs, ORC Jogs and calibration Iotas were reviewed for the period. February 2016 through
January 2017: No limit violations were reported and all monitoring frequencies were correct.
ORC must indicate on future DMRs all certified labs used for the sampling done for that
months DMR.

were not in service at the time of the inspection. The dewatered sludge is transported to the
on-site landfill equipped with a liner and leachate collection system.
The Ary ash and sluiced ash (removed from the ash basin treatment system) are also
disposed into the on-site landfill. The oypsurn material from the FGD system is reused W an
outside company (National Gypsum) in the dry wall market.
