On behalf of the owner, Faith Properties, we request modification of our prior (March 2008) PCN
submittal (Action ID SAW-2008-01215). Specifically, the applicant intends to construct the road
crossing as previously designed and approved including the construction of an arch (bottomless)
span in place of the recently proposed pipe/culvert (impact S-3). In doing so the applicant has
avoided the proposed impact of 1421f feet of jurisdictional stream. Originally an arch span was
specified at the location and included in plans approved with permit Action ID 200420295
(DWQ#5-1881). The applicant still requests proposed temporary impacts T-2 and T-3 as shown
in the prior submittal for temporary sewer construction. These impacts represent the relocation of
a proposed sewer crossing approved under a NW 12 permit (Action ID 200420295) but never
constructed. The applicant intends to follow regional and general conditions of NW 12 for the
sewer impacts. Lastly, we request approval of 141f of stream associated with the existing road
crossing at impact S-1. Based on our field review final construction resulted in
additional/extended riprap placement approximately 141f in excess of the approved 741f of
impact.
Plans for proposed impacts T-2/T-3 and revised impact S-1 were provided in our March 2008
submittal as well as with supplemental plans requested later. Please contact me if you have any
