Subject: Stormwater Permit Request for the widening of US 158 between from the Pasquotank
River to SR 1257 (Havenwood Drive) in Camden County. TIP No. R-2414A, Debit
WBS 34430.1.1 $505.

The North Carolina Department of Transportation (NCDOT) proposes to widen existing US 158 to a
four-lane facility from the Pasquotank River to SR 1257 (Havenwood Drive). Camden County falls
under the jurisdiction of the Coastal Area Management Act (LAMA). The NCDOT is applying for a
Clean Water Act (CWA) §404 Department of Army Permit, a North Carolina CWA §401 Water Quality
Certification, and a CAMA Federal Consistency Review from the N. C. Division of Coastal Management.
Application was previously made on June 28, 2004 for the R-2414 B section of this project.
Subsequently, Permit No. SW7040701 was issued on August 11, 2007.

A Stormwater Application Form, the Project Scope Narrative, two sets of project plans, and the authority
to debit $505 for the permit application fee are provided with this request. Please review this project for
authorization by your division.
