Gentlemen:

NCDOT recently (approximately 2 1/2 weeks ago) submitted a permit package to you for
SR 1521-A project. It was discovered after submission that the some of the
information provided on this project was actually from the Onion Mountain part of the
project and we advised that we would revise the permit and correct the mismatched
information.
It has come to our attention that the revised application was mailed on June 5 for SR
1521-A Mountain Grove Road (application is dated May 13, 2008). Unfortunately, due
to a misunderstanding between the Division Environmental Office and the County
Maintenance Office, this permit application also was submitted with some incorrect
information and retained some outdated application formats. David, I am advised that
we may have stopped the COE copy before it was sent, so you may not receive this.
Please disregard this revised application package, we will resubmit another permit
application package for this project. Hopefully, the third time will be a charm.
We apologize for any inconvenience and confusion that this has caused.
