Dear Dr. Holden:
The North Carolina Division of Water Resources conducted an inspection of the Laurel Elementary School WWTP
on February 24, 2017. This inspection was conducted to verify that the facility is operating in compliance with
theconditions and--lirnitations specified in-NPDES WW Permit No. NC0034207. A summary of.the findings and
comments noted during the inspection are provided in the enclosed copy of the inspection report.
The following violation(s) were noted during the inspection:
Inspection Area Description of Violation
Operations &Maintenance Rusted walkway grating, support brackets, and bar screen.
Part II. Section C.2 The Permittee shall at all times provide the operation and maintenance resources necessary to operate
the existing facility at optimum efficiency.
Remedial actions should have already been taken to correct this problem and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law.

On 2/24/2017, I (Dan follow up inspection at Laurel Elementary. Also present was
• Joe Davis,who is the maintenance supervisor. The aeration basin that was full of sand from flood
deposition had been cleaned out thoroughly. Joe was preparing the plant to be restarted.
I found walkway gratings, a support bracket, and bar screen supports that were completely rusted and
unsafe for plant operation. These items need to be repaired as soon as possible.

on. One bracket supporting the grating is rusted almost completely away.
