2011. The SWM package for Lot #1 was submitted back in June 2015 and put on hold. The O&M forms
were not submitted at that time, but are included herein.
Since this project falls within the overall approval for Airlie Industrial Park (Phase 1), we were instructed by
Mr. Alan Johnson that there is no application needed or fee. The prior approval # is referenced above. If

Laura Alexander forwarded your email to me. From our document filing system, it looks like the review was
discontinued after you indicated that the contractor wanted to hold off on providing a signed 0&M agreement
until it could be provided by the eventual property owner.
You can review all of the information on the project here.
You will need to re -submit a request for stormwater plan review. From a cursory review of the file, it would be
very helpful to provide a summary of the project's permitting and stormwater plan review history with the
request. It looks like a stormwater plan was reviewed and approved in 2014. 1 couldn't tell if this request is for
a modification of the previous approval or a plan for a new phase of the project. Please send the request to
Karen Higgins attention at the mailing address below and include the project number. No fee is required for
stormwater approvals.

Sand Filter Operation and Maintenance Agreement
I will keep a maintenance record on this BMP. This maintenance record will be kept in a log in a
known set location. Any deficient BMP elements noted in the inspection will be corrected, repaired or
replaced immediately. These deficiencies can affect the integrity of structures, safety of the public, and
the removal efficiency of the BMP.
Important maintenance procedures:
— The drainage area will be carefully managed to reduce the sediment load to the sand filter.
— The sedimentation chamber or forebay will be cleaned out whenever sediment depth exceeds
six inches.
— Once a year, sand media will be skimmed.
— The sand filter media will be replaced whenever it fails to function properly after maintenance.
The sand filter will be inspected quarterly and within 24 hours after every storm event greater than
1.0 inches (or 1.5 inches if in a Coastal County). Records of operation and maintenance will be kept in
a known set location and will be available upon request.
