Re. Winston Pointe PUD. Phase IA
Dear Holley:

Enclosed are three sets of plans for the Winston Pointe PUD phase 1A. erosion control plans,
one exccuted financial responsibility form, three sets of calculations, and a check for 5230.00.

Mr Steve Mitchell has already visited the site. No DWQ approvals are required for Phase 1A
construction Mr Kevin Martin with S&EC has informed me that Mr. Todd Tugwell with the
Corps of Engineers has verbally approved the (mly creek crossing that occurs for Phase IA. The
crossing; is eotisidered a ininor impact below notification threshold. Mr. Tugwcll's approval is
verbal, however if you need written verification, please advise.
