The Division has determined that your application is incomplete and cannot be processed. The application is on hold until all of the following items are addressed: [15A NCAC 02H .0507 (c)] The submittal indicates that a 42" HDPE pipe will be slip line installed into the existing 48" pipe. The contractor will then fill all void space with flowable fill and grout all cracks. Condition 5 of Water Quality Certification (WQC) 3883 requires that "All work in or adjacent to stream waters shall be conducted so that the flowing stream does not come in contact with the disturbed area... Approved best management practices such as sandbags, rock berms, cofferdams, and other diversion structures shall be used to minimize excavation in flowing water. Exceptions to this condition require application submittal to and written approval by the Division." In addition, Condition 9 of WQC 3883 requires that "If concrete is used during construction, then all necessary measures shall be taken to prevent direct contact between uncured or curing concrete and waters of the state. Please provide additional detail and construction sequences on how you plan to comply with condition 5 and 9 of WQC 3883. If necessary, ensure that temporary impacts associated with dewatering for the culvert installation are indicated in a revised impact table [15A NCAC 02H .0507 (c)] 2. The submitted plans show proposed rip -rap at the inlet area to assist in pipe stability. The proposed rip -rap is considered a permanent impact. Please update table 3 on page 4 of the PCN to include the rip -rap impact length. Per condition 15 of WQC 3883, "All rip -rap shall be buried and/or keyed in such that the original stream elevation and streambank contours are restored and maintained." Please provide a detail showing the proper installation (below the streambed elevation) of the rip -rap pad at the culvert inlet. [15A NCAC 02H .0507 (c)] 3. If all of the requested information is not received by March 23, 2017, the Division will be unable to approve the application and it will be returned. The return of this project will necessitate reapplication to the Division for approval, including a complete application package and the appropriate fee.
