WBS No. 17BP.3.R.53 Dear Mr. Shaver: Please find enclosed the preliminary jurisdictional determination (PJD) request, Low Impact Bridge Replacement (LIBR) package and permit drawings for the above referenced project that was prepared for NCDOT by HTNB and ESI. Project Description: The purpose of this project is to replace Sampson County Bridge No. 148 on SR 1434 over Mill Creek which is classified as C;Sw waters in HUC 03030006. Bridge No. 148 is currently 35 feet long with 253 foot deck width. The replacement structure will be an 85 feet long single span bridge with a 33 foot total deck width. The bridge will include two-ll-foot lanes and 4.0-foot offsets. The bridge length is set by hydraulic requirements. The approach roadway will extend approximately 238 feet from the southeast end of the new structure and 137 feet from the northwest end of the new bridge. The typical cross section of the approaches will include 2 — 11" asphalt paved lanes with 2 ft. paved shoulders and 6 ft. grass shoulders. A soil driveway will have to relocated from Sta. —L- 19+71.75 to 19+25 RT. The roadway grade of the new structure will be approximately the same as the existing grade. The proposed bridge does not contain deck drains and all stormwater from the bridge and approaches will be collected by a stormwater drainage system and discharged into a Class B rip rap pad at —L- 19+36 RT. Jurisdictional Imp acts: There is a mill pond upstream of the bridge and the discharge from the dam has create substantial scour on the downstream right bank therefore, 61 linear feet of Class II rip rap bank stabilization will be required on from Sta. —L- 18+92 to 19+01. Since the proposed slope is less than 1.5:1, NCDOT would like to a waiver from the filter fabric requirement so the rip rap can be pressed into the bank for better placement and stabilization. Minimal wetland impacts (0.01 acres of inechanized clearing and fill) are proposed in the southeast quadrant along the shoulder. Utilities: Based on the Preliminary Utility Analysis and Progress Report, the only utility conflict on this project is the SREMC powerline which will have to be shifted to the NNE. There are no jurisdictional impact associated with this relocation. Regulatory Approvals: Section 404 Pernut: We anticipate USACE Nationwide 3 and 13 will apply to this project. Written concurrence from the USACE is not required for this project. NCDOT will abide by all conditions listed in Nationwide 3 and include a copy of the certification in the permit package and/or contract. Section 401 Pernut: We anticipate 401 General Certification 3883 and 3885 will apply to this project. Written concurrence from the Division of Water Resources (DWR) is not required for this project. The existing bridge consists of 1 (Description of Minimization of Water @ 17' - 9" and 1@ 17' - 7" Reforced concrete deck on timber joists and timber piles and is to be replaced with a 1@ 40' - 0" and 1@ 45'-0", 21" cored slab bridge (85' total Quality Impacts) length, 33' total width) at the same location. The proposed bridge does not contain deck drains, all storm water from the bridge and approaches will be collected by a storm drainage system and discharged into a Class "B" rip-rap pad at -L- 19+36 on the right. There are no proposed ditches on this project. Furthermore, historic maps af Sampsnn County were appraised for former structure locatians, land use patterns, or other confirmatian of historic occupation in the project viciniry and archaeologicalJhistarical reference materials were re�iewed as welL In addition, tapvgraphic, �eningic, flaod bnundary, lidar� and NRGS soil survey maps were referenced for the e►►aluation of geomorphalagical. pedeological, hydrolagical, and other environmental-type elements that may ha�e resulted in past occupation at this location. Finally, review of aerial and an-grnund images {NCD�T 5patial �ata Viewer, GQogle, ARC-G15j afforded first-hand perspeetives af the overall study area which were useful for assessing locafized disturbances, ba#h natural and human induced, which compromise the integrity of archaeofogical sites/deposits, Based on the presen�e of a previously recnrded archaeol�gical site (315P235} an in-field reeonnaissance and survey of the site location is recommended prinr ta construction�replacement activities. This survey work will seek ta determine if any portion of 31SP235 extends int� th� APE and will e�aluate the resaurce for NRHP eligibiiity if cantained within the project baundaries. A recannaissance inuestigation of the project area was completed hy NCOQT archaeologists Seatt Hal�orsen and Damon lanes on Friday, Qetober 28, 2016. No partian of 31SP235 is located within the pre5ently defined APE. The mill stru�ture is located a caupfe of hundred feet from the eastern APE boundary. Also, there are no mill elements {mill race, dam, etc.} contained in the ARE. The milf dam, situated east af the APE, was breeched during Hurricane Mathew by the falling af a giant tree. The entire contents of the mill pond emptied over and through the dam flooc#ing the entire APE. During aur �isit, some two weeks later, e�idence of the fEaad was still �isible in the APE. 7he northern and southwestern quadrants still contained panded water, whike the southeastern quadrant is a natural spill- way for the dam. Because of these cnndtions, and since the APE Rs already underlain by frequently flooded and poorly drained soils {BHj, na shovel testing was necessary to confirm the absence af significant archaeological resnurces. No further arehaealogical wnr[c or consultatian will be needed for the prc�ject as prQposed. Descri�tion ofreview activities, results, and conclusions: HPOWeb reviewed on 1 November 2016 and yielded two SS and no NR, SL, DE, or LD properties in the Area of Potential Effects (APE). The APE for historic architectural resources extends 300 feet from either end of the existing bridge (NW-SE) and 150 feet to either side of the SR 1434 (Huntley School Road) centerline (NE-SW) to encompass all anticipated construction activities and likely impacts. Sampson County current GIS mapping, aerial photography, and tax information reveal a mostly wooded APE with several residential and agricultural resources dating mainly from the 1920s to the 2000s. The circa-1922 Robert Crumpler House (SP0511) sits approximately 275 feet NE of the SR 1434 (Huntley School Road) centerline, beyond likely project impact; no other buildings or significant landscape elements appear on the property, which, however, may be related to mill resources located further to the SE (#1324 Huntley School Road, PIN: 06064162001). The APE intersects a parcel on which an unexceptional, circa-1940 house is located about 480 feet NW of the existing bridge (#1407 Huntley School Road, PIN: 06065220006). SE of the bridge, the APE intersects parcels (PINs: 06064220009 and 06�05582003) containing two unexceptional houses and various outbuildings dating to the first half of the twentieth century, a cemetery located about 350 feet NE of the SR 1434 (Huntley School Road) centerline, and the above-ground remains of a mill, dam, mill race or sluiceway, and mill pond, all but part of the race/sluiceway outside the APE approximately 290 feet and more E of the bridge. The mill resources are recorded in the state architectural survey as the Rob Crumpler Mill (SP0512); they appear in the state archaeological record as Matthew's Mill (31SP235). There are no National Register-listed properties in the APE or along the proposed off-site detour route. Initial screening of the project suggested that additional investigation of the mill resources might be needed (survey required form, 7 November 2016), but additional design and permit information received from Division 3 indicates that a finding of "no historic properties affected" will satisfy both Section 106 and GS 121-12(a) compliance requirements.
