WATER QUALITY WETLANDS AND STORMWATER BRANCH Subject: Request for Additional Information Hartley Drive Widening and Extension City of High Point, Guilford County, NC NCDWQ #07-1995 USACE Action I.D. #2006-20795 RWA Project No. 3202000 Dear Ms. Homewood: On behalf of the City of High Point, STV/Ralph Whitehead Associates, Inc. (STV/RWA) is submitting this letter of intent to submit the additional information that was requested by the Division of Water Quality (DWQ) in a letter addressed to Mr. Keith Pugh of the City of High Point dated June 16th, 2008, after you received our additional information on May 6th, 2008. This most recently requested information is anticipated to be prepared by the middle of August. Therefore, we request that the DWQ keep this project file open in anticipation of these permit materials. The City of High Point has proposed extending and widening the existing Hartley Drive from US 311 (North Main Street) approximately 6,000 feet to the west, to the intersection of Westover Drive and Shadow Valley Road. The City of High Point has contracted STV/RWA to be the acting agent and perform wetland-permitting services for this project that is called "Hartley Drive Widening and Extension". The following information will be submitted to the DWQ as soon as it has been prepared: 1. For the three proposed Dry Detention Basins please submit the BMP Supplemental Forms and O&M Form. These forms can be found on the Divisions an employee-owned company providing quality service since 1912 r J STV/Ralph Whitehead Associates website: http://h2o.enr.state.nc.us/su/bmp forms.htm. Please ensure that all items noted on Worksheet #2: Checklists are clearly identified on the engineering plans. 2. The EEP mitigation acceptance letter submitted with the application is no longer valid. Please provide anew EEP mitigation letter. 3. Please provide construction details for the boulder (toe protection) installation and the cross vane installation with the stream relocation/restoration area. 4. Please provide a planting plan for the stream buffer on the north side of the channel that includes the standard planting density as indicated in the Stream Mitigation Guidelines document found on the USACE website: hyp://www.saw.usace.army.mil/WETLANDS/Mitigation/stream mitigation.html. The Division acknowledges that planting density will not be feasible in the riparian buffer between the stream and the roadway, however some tree and shrub vegetation will be required to be planted immediately adjacent to the stream bank. As stated previously, we intend to submit the requested materials to your office before August 15th, 2008.
