Comment: The current permit was issued on March 3.2015,with an effective date of April 1. 2015. The
permit is set to expire on January 31,2019. The last compliance evaluation inspection was
completed on December 2.2010. The Division granted a waiver of the monthly reporting
requirement and operator visitations on May 26, 2009.

Comment: Since March of 2006 the permittee has pumped the effluent to the Long Creek VWVTP
instead of directly discharging into the unnamed tributary to Lonq Creek. The WTP is a
permitted industrial user(1064) under the City's pretreatment program and regularly
samples the effluent under this program.
