Directions to Site: From the junction of NC 150 (west)and E. Maiden Road,take a right onto E. Maiden Rd. and travel for 7.9 miles until merging onto E. Main Street. Continue traveling 1.7 miles and turn left onto S E Avenue for 0.2 miles and turn right onto W Finger Street for 0.2 miles. From W. Finger Street, continue onto W. Finger Saint Ext. and the WWTP will be located at the end of this road(0.9 miles). 6. Discharge Point(s), List for all discharge Point: Outfall 001 Latitude: 35°34' 35" Longitude: 81° 14' 26" See USGS Map included with the renewal application for specific location of the outfall. River Basin and Subbasin No.: 03-08-35 PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. a. Volume of wastewater and description of wastewater source(s)of each outfall: Page Two Outfall 001 — 1.0 MGD—domestic and industrial (1 SIU/CIU). The existing WWT facilities consist of mechanical screening; flow measurement; grit removal; influent lift station;two SBR tanks equipped with aeration and decant systems; post equalization tank; liquid disinfection; chlorine contact chamber; liquid dechorination;two aerobic digesters; and standby power. b. Pretreatment Program: Approved PART III -OTHER PERTINENT INFORMATION 1. Special monitoring or limitations(including toxicity)requests:None Requested. PART IV - EVALUATION AND RECOMMENDATIONS The WWTP appeared to be properly operated and well maintained at the time of the inspection.
