Comment: The records reviewed during the inspection were organized and well maintained. Discharge
Monitoring Reports (DMRs)were reviewed for the period November 2014 through October
2015. No effluent limit violations were reported and all monitoring frequencies were correct.
The ORC will resubmit an amended September 2015 DMR to correct the effluent zinc value
(not converted from mg/L to ug/L) and the influent BOD and TSS values on 9123115 and
9128115. The ORC will also review and resubmit(if applicable) any 2015 DMRs that had
reported an incorrectly transcribed effluent zinc value.

Comment: On-site field analyses (pH total residual chlorine dissolved oxygen,temperature) are
performed under the Town's field laboratory certification#5532. Water Tech Labs(BOD,
TSS ammonia fecal coliform,total phosphorus total nitrogen)and R&A Labs(toxicity
selected metals priority pollutants) have also been contracted to provide analytical support.
The laboratory instrumentation used for field analyses appeared to be properly calibrated
and documented. The facility staff must ensure that the calibration times are properly
documented in the calibration logs and the annual curve verification on the TRC meter is
performed every twelve months.

Comment: The subject permit requires composite BOD and TSS influent sampling. The influent
composite sampler was collecting time-based composite samples (one sample per hour)
without prior Division approval_ The ORC did not know how long the sampler was collecting
time-based composite samples; however, the sampler was reprogrammed to collect flow
proportional samples during the inspection. Please be advised that the subject permit
requires flow proportional composite samples unless prior approval is obtained from the
Division.
The CIRC and staff must ensure that periodic aliquot verifications(recommend monthly)on
the sampler are performed and documented.
