On July 17, 2015, Autumn Romanski, Cheng Zhang, and Erin Deck with the North Carolina
Division of Water Resources conducted a Compliance Evaluation Inspection of the Shearon
Harris Nuclear Power Plant and Environmental Center Wastewater Treatment. This inspection
was conducted to verify that the facility is operating in compliance with the conditions and
limitations specified in NPDES Wastewater Discharge Permit No.NC0039586. The time and
assistance of Duke Energy Staff-Robert Wilson, Gordon Codwell, Christian Hooft, Daniel
Johnson, Donald Safrit, Della Allen, Amanda Hair, and AQWA consultant Steve Barry was
greatly appreciated.
A summary of the findings and comments noted during this inspection and a copy of the
complete inspection report titled "Water Compliance Inspection Report" are provided for your
review.
The Raleigh Office received the requested flowmeter replacement and calibration information
on August 5th, and we received the sludge hauling record requested on August 20`h , on
August 20'hen satisfaction of pending items to complete this report. No further response to
this inspection is required

Summary of Findings /Comments
1. Permit/Special Conditions:
Your NPDES Individual Wastewater permit became effective March 1, 2007 and expired on
July 31, 2011. The facility is not currently under a Special Order by Consent.
The permittee completed the permit renewal application as required(6 months before
expiration in July 2011.) A new permit has not vet been issued, therefore this compliance
inspection is based on the 2011 expired permit that continues in effect until a new permit is
issued by the Division Of Water Resources (DWR).
The plant is required to submit an annual Biological Monitoring Plan for Harris Reservoir
and monitoring results are provided to the Division of Water Resources, Environmental
Science Section (ESS) in keeping with Federal Regulation 316 B. The plant is also required to
complete monitoring associated with the federal regulations as set forth by the US Nuclear
Regulatory Commission.
The NPDES Stormwater permit requirements were included in the wastewater permit standard
conditions issued with the permit in 2007. The NPDES Stormwater Permitting and
Compliance Unit now reside with the Division of Energy, Mines, and Land Resources (NC
DEMLR) therefore stormwater was not evaluated during this NPDES Wastewater Inspection.
The day of this inspection, July 17, 2014, Duke Staff explained that the old package plant
listed in the permit for (Outfall 002-treatment for domestic waste) was being refurbished and
down for maintenance. A new AQWA wastewater treatment had been installed. Please ensure
that the DWR NPDES Permitting Unit is updated with the AQWA treatment units. Also,please
list any changes that occur to the old package plant treatment units during the
refurbish/maintenance project so that these treatment units can also be accurately listed in
new permit when issued.
2. Records/Reports:

a) A review of the daily monitoring data submitted during the previous 12-month period
and a review of the permit conditions indicates the facility was in compliance with the
permit requirements.
The facility has had no violations within the last 12 months of review (DMR's dated
March 2014—March 2015).

The Wastewater DMRs from July 2014 and December 2014 were reviewed with no
discrepancies noted
The Division of Water Resources (DWR) implemented an electronic discharge monitoring
report program. This program requires all NPDES-permitted facilities report monthly
monitoring report data electronically through the Division's eDMR(Electronic Discharge
Monitoring Report)program. All existing permitted facilities and any new permitted
facilities/dischargers will be required to comply with the permit requirement no later than 90
days after the next permit issuance becomes effective. For registering for eDMR submittal and

b) The facility must designate a certified Operator-in-Responsible-Charge (ORC) having
a certification level equal to or greater than the facility classification. A Back-up ORC
must also be designated with a minimum certification level at least one level less than
the facility classification.
The facility is currently in compliance with these requirements. This facility treatment
system(s) are classified Wastewater I, Wastewater II and Physical Chemical (PC-2).
The Operator(s) in Responsible Charge are: Robert Wilson WW-2 and PC-2 (Cert. No.
24063 and 28914); Della Allen WW-3 and PC-2 (Cert. No. 22916 and 27903); with
Back up ORC's: Christian Hooft WW-3 and PC-2 (Cert. No. 15163 and 28910);
Gerald Boley WW-2 and PC-2 (Cert. No. 7801 and 29297); Robert Vickers WW-2 and
PC-2 (Cert. No. 991179 and 985282 ; David Allen WW-1 (Cert. No. 11001082) and
Penet Melton WW-1 (Cert. No. 9910810).
The facility is currently in compliance with these requirements.
3. Self-Monitoring Program:
You are reminded that your monitoring samples must be collected, preserved, and analyzed by
appropriate procedures and methods. You should maintain records of the sampler temperature,
and a certified laboratory must calibrate the sample thermometers annually.
Your operator is reminded that he/she must keep a logbook of all operation/maintenance
activities undertaken at the facility. The logbook should include all daily process control
activities including any field measurements conducted for process control. A visitation log for
the facility must also be maintained.
The records reviewed the day of this inspection show the facility was in compliance with
these requirements.

4. Laboratory:
For all analytical data submitted for compliance monitoring purposes, a certified laboratory
must be utilized for all laboratory analysis. Additionally, if field parameter testing is
conducted, an operator certified to perform the specified field parameter analysis is also
required. The operator needs to be sure to calibrate all meters/instrumentation before each use
and retain all calibration records.
The facility is currently in compliance with these requirements. The facility uses Pace
Analytical Commericial Certified Labs and the Environmental Center holds a current NC
DWO Laboratory Field Parameter Certification #5145 (for outfall 007)for Temp, DO,pH,
and TRC measurements.
The Regional Field Lab Inspectors checklist was completed during the 2012 NPDES
Wastewater Inspection. The TRC HACH 10014 Method and Lab equipment were
examined during this inspection, but time constraints to complete other inspection items •
prevented further examination of field lab operations. Lab inspections are also performed
separately by the DWR Lab Certification Program.
5. Flow Measurement:
If the permit requires a flow measurement device for determining the effluent flow, the
flow measurement device must be calibrated annually.
This facility is required to monitor and report flow. Duke staff explained NPDES effluent
outfall 002 (Old Package Plant — flowmeter is out - of service for plant
rehabilitation/maintenance. Since the 002 flow is being batch processed through the new
AQWA treatment plant, the flow being reported is flow measured by an inline flow meter at
the end of the AQWA Treatment trane rather than by influent pump logs. This meter was
not working properly and had not yet been calibrated the day of the inspection. The
facility was able to perform fixed time fixed volume checks on the flow to meet
proportional flow requirement for composite sampling at this outfall. The flow meter
service company had already been scheduled to complete the flow meter repair the next
week. On August 5, DWR received notice that a new flow meter was placed in service and
calibrated
6. Effluent/Receiving Waters:
The effluent discharge should be clear with a continuous flow with no visible changes to
the receiving waters.
The effluent discharge from Outfall 007 was not discharging at the time we inspected it.
The effluent discharge from Outfall 006 is underneath the Harris Reservoir and not visible
from land.
7. Pretreatment:
This facility currently has no pretreatment permit and does not accept or discharge
domestic or industrial waste to a sanitary sewer system.

8. Solids Handling/Disposal:
The facility must dispose of solids by an acceptable method such as a landfill or utilize the
services of an approved solids hauling and disposal contractor, which requires the facility
to be included under the contractor's non-discharge permit (i.e. land application). The
facility is reminded to maintain adequate records (i.e. shipment dates, quantities, etc.) of
any solids hauling and/or disposal activities.
The facility utilizes Granville Farms and holds WQ0000838 Residuals Permit. The facility
also utilizes some on-site frac tanks (8-the day of the site visit) and Shamrock Septic
Haulers to manage wastewater on-site during outages and for use as needed during the
current rehabilitation and maintenance at the old package plant. The HNP Sewage
Treatment Plant Sludge is stabilized on-site to meet pathogen and vector reduction —
sludge sampling is completed by Pace Analytical/A &L Eastern Laboratories.
It was explained that Sludge/Solids in the settling basins at the Environmental Center have
not been removed, and this system has been operated since installation in 1995 as
facultative lagoon system. The facility reported maintenance is needed and staff are in the
process of evaluating options and looking at more current treatment technology-possibly
an upgrade to new AQWA treatment unit at this location. -
9. Operations & Maintenance:
The facility was not in operation as originally permitted during this site visit. However,
DWR has received notice of the current operations, but the permit has not yet been updated
to reflect the current operations. The facility was also conducting rehab and maintenance of
the 002 old package plant. Optimum operations during maintenance remain requirements of
the NPDES permit and the facility was able to meet operational requirements with the new
AQWA plant that was prior installed and placed in service.

The permittee was compliant with the permit requirements reviewed the day of this inspection.The
original site visit for this inspection was conducted on July 17, 2015. Subsequent paperwork requested
on flow meter was received on August 5, 2015. Additional information requested on sludge records
was completed on August 20,2015.
DWR appreciates the permittee's efforts and investment in the collection line repairs, the old package
plant refurbishment, and the installation of the new AQWA Treatment Plant at Outfall 002. As
discussed at the site visit, the permittee is also looking at needed maintenance and technology
upgrade options(possibly new AQWA Treatment Plant)at the Environmental Center. The old settling
basins are more labor intensive, produce more sludge, and can be difficult to batch operate. The clay
lined settling basins installed in 1995, are a 1970-1980's technology ready for evaluation and
consideration of current treatment needs and newer technology, as feasible.

RRO was already notified of operational start-up- No A to C was required for the
construction of the AQWA wastewater treatment system.

Comment: The influent is pumped from the main plant footprint to the old package wastewater
treatment plant. The influent line was recently replaced with HDP pipe to help remedy
leaking/increased TSS loading to the package plant-issues experienced in the past.
Influent was being held in reconfigured aeration basins in the old package plant(being used
for storage)the day of this inspection and influent was being re-routed/processed at the new

batch feed of wastewater contained on-site(the permittee provided a record/schedule of
frac tank burndown from May 2015(26 tanks to 8 tanks). Current operations and
maintenance dictate continued use of some frac tanks in order to complete old package
plant refurbish and maintenance. Main Plant Shutdown schedules can also impact
operations at the package wastewater plant-but the facility was not under a shutdown the

sludge with decant recirulatinq back through the wastewater treatment system.
It was explained that the new AQWA system has significantly reduced sludge production
compared to the old package plant operation.

The facility is reviewing maintenance of the settling basins and possible installation of a new
AQWA system at this location.

Comment: The day of the inspection the 2014 flowmeter at the new AQWA treatment unit was not
working, and a service visit was scheduled in July for repair/replacement and calibration.
*Subsequent paperwork was provided following the inspection confirming install, calibration,
and new operational flowmeter has been placed on-line.
Flow can be estimated at internal outfalls as the permit is currently written, but composite
sampling is required at outfall 002, 006, and 007.The composite sampling requires
proportional flow and it appears that the facility is in compliance as it was reported they are
using fixed time fixed volume as defined in the permit conditions.

Comment: The records were in excellent order. DMRs for July and December 2014 were reveiwed with
no discreptancies.
The facility holds Field lab certification#5145 for Temp, pH, TRC, and DO. Additional
investment has been made to ensure capability to-run TRC accurately using the titrated
method to eliminate any issues with Mn interference. This is the preferred method in this
case.The facility utilizes Pace Analytical certified Labs.
