Duke Energy Progress, LLC (Duke Energy) is submitting herewith supplemental information in
support of the NPDES renewal application submitted in 2011 for the subject facility. Please
include this supplemental information in those packages for your review.
The areas of wetness in the table below were discovered since the last permit application update
and were sampled, where determined necessary and possible, in accordance with the Riverbend
Plan for Identification of New Discharges (8 May 2015 version). Attached are the analytical data
for these areas of wetness as well as a map that indicates the locations of these areas of wetness.

This AOW is in an area of rip rap and gravel at the toe of the
Mayo S-01 A 36.538903 -78.89351 west side groin ditch of the ash basin dam. During the April
2016 sampling event,this area was not flowing.
This AOW is located to the southeast of S-03 on a hillside
36.538422 -78.890395 between two topographic draws.The water pools in two places
Mayo S-10 and continues flowing across the access road that leads to
compliance wells CW-2 and CW-2D before draining into
Crutchfield Branch which then flows northeast toward Mayo
Lake Road.
