Please review the Flows for the November 2015 DMR and submit a written response within 10 days of receipt of
this letter to this Office explaining why the Monthly Average Flow exceeded the permit limit by 296.99 %.
It was also noted that the Compliance Box on the back of the DMR was marked "Compliant"when in fact
it should have been marked "Non-Compliant".with an explanation for the violation.

Remedial actions, if not already implemented, should be taken to correct any noted problems. The Division of
Water Resources may pursue enforcement actions for this and any additional violations. '
