I. General Information
Facility System Name: Polk Central Elementary School
NPDES Permit No. NC0033553
Responsible Entity: Polk County Schools

Description of treatment process: Polk Central's Wastewater Treatment is a Class I Facility
with the capacity of treating 0.010 MGD with the following components:
• 3000-gallon grease interceptor adjacent to the cafeteria with a 3000 GPD effluent filter
• 11,000-gallon septic tank
• 3000-gallon dosing tank with duplex alternating siphons(3000 gallons per dose)
• 1500-gallon tank with a 5000 GPD effluent filter
• 3200-ft2 surface sand filter with low pressure distribution
• Tablet chlorinator
• 5000-gallon chlorine contact chamber
• Tablet dechlorinator
Effluent is discharged from said treatment works into the South Branch Little White Oak Creek
In sub-basin 03-08-02 of the Broad River Basin in accordance with effluent limitations,and monitoring
requirements.
The Facility is located at the Polk Central Elementary School's WWTP, 2141 South NC Highway 9, east of
Mill Spring in Polk County.
II. Plant Performance
Monitoring Violations: None
Reporting Violations: None
To the best of my knowledge there were no environmental impacts due to plant operations or operator
error.
III. Notification
This report has been made available to the taxpayers of Polk County and Polk Central Elementary
School. This Annual Performance Report has been advertised in the two local area papers.Copies of this
report will be available at Polk County Board of Education and Polk Central Elementary School during
regular business hours.

IV. Certification
