REQUEST FOR ADDITIONAL INFORMATION
Dear Mr. Mayo:
On May 21, 2008 the Division of Water Quality (DWQ) received your application dated
7, 2008, to impact 0.049 acres of wetlands and protected riparian buffers with fill to constru ;-' L.11
proposed bulkhead. The DWQ has determined that your application was incomplete and/or
provided inaccurate information as discussed below. The DWQ will require additional
information in order to process your application to impact protected wetland, streams and/or
buffers on the subject property. Therefore, unless we receive the additional information
requested below, we will have to move toward denial of your application as required by 15A
NCAC 2H.0506 and 2B .0233(8) and will place this project on hold as incomplete until we
receive this additional information. Please provide the following information so that we may
continue to review your project.
Additional Information Requested:
1. Justification of Proposed Impacts
It is required that the applicant shows justification of need for the proposed bu!k'-
in the CAMA Field Investigation, and confirmed in a site visit, the area of the props.', -
did not show any appreciable erosion.
2. Avoidance and Minimization of Impacts
It is required that the applicant demonstrates avoidance and minimization of wetland impacts in
design of the proposed project.

Please note that is required that Zone I and Zone II of the Neuse Riparian Buffer be restored
tJ.:ll 61d of the proposed bulkhead. It is necessary that a Buffer Restoration Plan be submitted to
:s Office for approval. The restoration plan should include the following items:
(i) A map of the proposed restoration site (drawn to scale).
(ii) A vegetation plan. The vegetation plan shall include a minimum of at least two
native hardwood tree species planted at a density sufficient to provide 320 trees per
acre at maturity.
(iii) A grading plan. The site shall be graded in a manner to ensure diffuse flow through
the riparian buffer.
`'v! A fertilization plan.
(v) A schedule of implementation.
(vi) Within one year after the Division had approved the restoration plan, the applicant
shall present proof the Division that the riparian buffer has been restored. If proof is
not presented within this timeframe, then the person shall be in violation of the
State's riparian buffer protection program.
Please respond in writing to the DWQ within four weeks of the date of this letter to with the
2q _lested information. If we do not hear from you within four weeks, we will assume that you no
longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Roberto Scheller of the Washington
