This concerns the subject high hazard dam recently modified pursuant to issuance of an
Approval to Install Instrumentation on March 24, 2015 as required by the Dam Safety Law of
1967. Record Drawings and the engineer's certification were received on May 11, 2015. The
dam was certified by Mr. David D. Wilson, PE. An inspection of this dam was made by Land
(duality Section staff of the Asheville Regional Office on June 12, 2015. Prior to issuance of an
approval of the modifications, additional information or revision is requested for the following
items and corrected as -built drawings must be submitted:
1. A) On Page 3 of the engineer's report, it states that the wells were placed "approximately
10 feet" apart, however, field observations found that the wells were spaced
approximately 6 feet apart. B) One borehole location was submitted on the as -built
drawings to represent all of the wells that were installed. C) The as -built drawings were
not sealed by a Professional Engineer. Please revise the engineer's report accordingly,
submit an as -built drawing that indicates the locations of all wells, and have a
Professional Engineer seal the as -built drawings.
2. On Page 4 of the Engineer's Report, it states that several different geotechnical
observations were made including the flow discharging from the toe drains during the

installation of the wells, however those have not been submitted. Please submit
geotechnical field observations including observed flow discharging from the toe drains
and photographic documentation of the dam conditions.
It would greatly aid further application review if a response to each of the above comments were
made in a cover letter submitted with two sets of the revised drawings. You should contact Ms.
Melissa Ding, PE, Assistant Regional Engineer in the Asheville Regional Office when the above
