Box 879 Claremont, NC 28610-0879 Date: December 16th, 2016 RECEIVED Attention: DWR Central Files i\N 3 p 2017 Division of Water Resources 1617 Mail Service Center Ctr`'•TFILES Raleigh, NC 27699-1617 rrVl 101`1 Subject: Stormwater DMRs COMMSCOPE — CLAREMONT FACILITY NCDWQ General Permit No. NCG030000 Certificate of Coverage No. NC 03065'0' December 2016 Stormwater DMRs To Whom It May Concern: I have enclosed two (2) copies of the December 2016 Stormwater DMR for CommScope, Inc. WWTP. The CommScope Certificate of Coverage No. is NCG030650. For this month, CommScope - Claremont completed a wet sampling event of all three (3) outfalls for the facility on Tuesday, December 6th, 2016. All three (3) outfalls were monitored for oil & grease, TSS, pH, lead, copper, and zinc. For this sampling event, the following exceedances from the General Stormwater Permit were noted: Outfall # Copper (0.007 mg/L) ' Zinc 0.067 ms/L) N/A i ®D129 --''Q -- N/A ! @j39 From the above, it is noted that all three (3) outfalls were above the benchmark exceedance for zinc (0.067 mg/L). Outfall 003 was above the benchmark exceedance for copper (0.007 mg/L). Due to these exceedances, the Stormwater Pollution Prevention Team (SWPPT) will be implementing the following corrective action steps: 1) Perform increased sampling events to further investigate these exceedances; 2) Perform monthly dry weather inspections during months of no discharge; and 3) Follow up with necessary Best Management Practices as identified in the Stormwater Pollution Prevention Plan. As the above corrective action steps are implemented, the SWPPT will work on addressing the causes for these exceedances. Furthermore, the SWPTT will continuously follow up with necessary Best Management Practices as needed from henceforth. Please feel free to call me at 828-459-5153 to discuss this information further. For purposes of this permit the definition of Total Toxic Organics is that definition contained in the EPA Effluent Guidelines for the facility subject to the requirement to sample (for metal finishing use the definition as found in 40 CFR 433.11;for semiconductor manufacture use the definition as found in 40 CFR 469.12; for electronic crystal manufacture use the definition as found in 40 CFR 469.22; and for cathode ray tube manufacture use the definition found in 40 CFR 469.31). The solvent management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities electing to employ the TTO monitoring waiver, the discharger shall sign the following certification statement: "Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics (TTO), I certify that to the best of my knowledge and belief,no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing the all the provisions of the solvent management plan included in the Stormwater Pollution Prevention Plan." i i/b1 e e WI.
