On November 17, 2016, Roberto Scheller of this Office conducted an inspection at the subject
facility. This inspection was conducted as a Compliance. Evaluation Inspection (CEI) to insure
compliance with permit requirements and conditions. At the time of inspection facility appeared
to be well maintained and operated. We wish to .thank you and the operating staff for assistance

Comment: Effluent pipe not discharging at time of inspection. The subiect facility- has been discharging
to the Town of Norwood collection systern. The last DMR submitted to the Division was on
May 2012. DMR reporting requirements were waived on June 14, 2012.
Please be advised that in accordance with your current NPDES Permit # NC0088676, Part
Ii. Section C. Item 2. Proper Operations and Maintenance, " The permitee shall at all times
Properly operation and maintain all facilities and systems of treatment and control (and
related appurtenances) which are installed Or Used bV the Permittee to achieve compliance
with the conditions of this permit." It is therefore necessary that all .equipment used for
discharging to out outfall 001 be maintained and operational in the event discharge out outfall
001 is necessary.
