The information to be sent to NCDEQ, including a sample cover letter, is included in Appendix 1. The originals and one (1)copy of only the following items are required to be sent to NCDEQ: • Completed and signed May 2016 Stormwater Discharge Outfall (SDO)Monitoring Report; • Ensure that the date submitted section at top of the form is completed before mailing All forms are to be signed by the company signatory authority in the places indicated. Mail the original and one(1)copy of the signed information by certified mail to NCDEQ at: Division of Water Quality ATTENTION: DWQ Central Files 1617 Mail Service Center Raleigh,North Carolina 27699-1617 2 IV ACTIONS In accordance with the discussions with the NCDEQ, Central Office representatives,the facility is required to continue monthly monitoring for three (3)consecutive months and report the results of the sampling events at the conclusion of the third sampling event to the NCDEQ Stormwater Program (to the attention of Mr.Ken Pickle). With two (2)of the required three(3) sampling events having been completed (albeit not consecutively due to intensity of precipitation events providing an actual discharge from the facility stormwater pond outfall SW001),the one remaining event should be conducted similar to all other sampling events in that all parameters (not just metals)need to be analyzed in conjunction with visual stormwater monitoring and other associated inspections (including tier inspections to be conducted within 2 weeks of receiving this package)as required by the General Permit. Clean and remove debris from the stormwater pond and the outfall pipe as required and continue the routine cleaning schedule implemented for the stormwater pond and outfall areas to minimize the potential for benchmark value exceedance at Outfall SW001. Conduct maintenance and calibration of the pH meter according to facility ISO protocols and meter manufacturer instructions. Ensure that the calibration liquid is not used past its expiration date. Ensure that the sampling personnel are wearing clean latex gloves while sampling and that samples are collected from the middle of the stormwater pond outlet pipe so as to minimize the amount of sediments collected in the sampling container. The pH of the stormwater sample and the rain gauge should be measured during the sampling event in the field to ensure that the sample pH outside the benchmark range is not due to the actual pH of the precipitation. In addition,due to the close proximity of the stormwater outfall to the unnamed tributary to Steele Creek, ensure that samples are collected only from the stormwater pond discharge to avoid any cross contamination which could lead to benchmark exceedance. There have been reported instances of the velocity of the tributary stream flow pushing water back into the discharge pipe which could lead to cross contamination and this should be avoided at all costs. If such a situation exists, stormwater samples should not be collected as this will not be representative of the facility stormwater. Complete the Stormwater Event Data form such that the rainfall record from rain gauge is observed and noted when samples are collected and when the precipitation event has ended. Rainfall amounts during sampling events are required to be submitted to NCDEQ through the DMR form and this will allow an accurate record of the measureable storm event. This is especially critical in lieu of the monthly sampling requirements to obtain relief from the Tier III status of the facility. In addition ensuring that daily rainfall (including no rainfall) is documented in the Daily Rainfall Log will provide documentation to validate no discharge scenarios when stormwater sampling could not be conducted for the required monitoring period. Review the past stormwater sampling report memos for additional action items for maintaining the metals in the facility stormwater under benchmark concentrations for source controls. V RECORDS The results of stormwater monitoring must be included in your facility's Stormwater Pollution Prevention Plan (SWP3). All materials to be included in the SWP3 are attached in Appendix 2. Included are: 3 • May 2016 Stormwater Discharge Outfall Qualitative Monitoring Report; • May 2016 Stormwater Discharge Outfall Analytical MonitoIcing Report; (Sign and place in Annex) • Laboratory Analysis Report; t;' • Storm Event Data Record • Annual non-stormwater discharge assessment and certification Place a copy of the stormwater records at the end of Annex F of your facility's One Plan.
