Directions to Site: From the junction of Interstate I-40(west) and Exit 125 (Lenoir Rhyne Blvd.), take a right onto Lenoir Rhyne Blvd. for approximately 1.3 miles and turn left.on 1St Avenue SE. Travel approximately 0.5 mile and turn right on NC 127 (North)and travel approximately 4.8 miles before turning right on Cloninger Mill Road. The WWTP entrance will be on the right following 0.1 mile. 6. Discharge Point(s), List for all discharge Point: Outfall 001 Latitude: 35°47' 54" Longitude: 81° 18' 00" See USGS Map included with the renewal application for specific location of the outfall. USGS Quad No.: D 13 SE 7. Receiving Stream or Affected Surface Waters: Catawba River(Lake Hickory) a. Classification: WS-IV &B b. River Basin and Subbasin No.: 03-08-32 PART II-DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. a. Volume of wastewater and description of wastewater source(s) of each outfall: Page Two Outfall 001 —6.0 MGD—domestic and industrial (5 total SIUs with no CIUs). The existing WWT facilities consist of mechanical screening; flow measurement; influent pump station; grit removal; dual oxidation ditches each equipped with an anoxic and anaerobic basin;three secondary clarifiers; gas chlorination; chlorine contact chambers; gas dechlorination;two gravity belt thickeners; aerated sludge holding basins(former aeration basins);odor control (biofilter, carbon absorption, ozone); and standby power. b. Pretreatment Program: Approved PART III -OTHER PERTINENT INFORMATION 1. Special monitoring or limitations(including toxicity)requests:None Requested. PART IV -EVALUATION AND RECOMMENDATIONS The WWTP appeared to be properly operated and well maintained at the time of the inspection.
