Enclosed is your first semiannual Stormwater Discharge Outfall (SDO) Monitoring Report, a copy of the
analytical results provided by Test America, and a copy of the Chain of Custody.
You are required by the permit to submit your SDO Monitoring Report to the Division of Water Quality
(DWQ). A cover letter to the DWQ for transmittal of the SDO Monitoring Report is also attached here for
your convenience. Please print the cover letter and the SDO Monitoring Report, sign and date inside the
gray boxes of the cover letter, and sign and date the certification sections on the second page of the SDO
Monitoring Report(note: you will need to sign both the Solvent Management Plan certification on the top
of the second page, and permittee certification the bottom on the second page of the SDO Monitoring
Report). Make a photocopy of the signed documents and place them in the monitoring section of your
SPPP. The original signed cover letter and SDO Monitoring Report should be sent via certified mail with
a return receipt or via Fed-Ex to the DWO at the address shown on the cover letter by July 8th,2016. You
do not need to submit the lab report to the DWQ.

MONITORING REPORT
Facilities that incorporate a solvent management plan into the Stormwater Pollution Prevention Plan may request that DWQ waive monitoring of total toxic organics. The solvent
management plan shall include a list of the total toxic organic compounds used and the other elements listed in the General Permit. For those facilities allowed such a waiver,the
discharger shall sign the following certification statement:

"Based upon my inquiry of the person or persons directly responsible for managing compliance with the permit monitoring requirement for total toxic organics(TTO), I
certify that to the best of my knowledge and belief, no dumping of concentrated toxic organics into the stormwater or areas which are exposed to rainfall or
stormwater runoff has occurred since filing the last discharge monitoring report. I further certify that this facility is implementing all the provisions of the solvent
management plan included in the Stormwater Pollution Prevention Plan."
