Cc:Wanucha, Dave Subject:Request for Additional Information: Horse Pen Creek Rd. widening, Greensboro, Guilford Co., NC; SAW-2013-01953 John, Thank you for your PCN and attached information, dated 10/28/2016 (received 1/27/2017 via email), for the above referenced project. I have reviewed the information and need clarification before proceeding with verifying the use of Nationwide Permit 14 http://www.saw.usace.army.mil/Portals/59/docs/regulatory/regdocs/NWP2012/NWP14_3-23.pdf. Please submit the requested information below (via e-mail is fine) within 30 days of receipt of this Notification, otherwise we may deny verification of the use of the Nationwide Permit or consider your application withdrawn and close the file: 1) Is dewatering proposed as part of the temporary impacts necessary to remove the Horse Pen Creek culvert? If so, please ensure that these impacts occur within the proposed temporary impact footprint, and include the method of dewatering. If dewatering is not proposed, please provide information on how turbidity will be controlled within and downstream of the project area during culvert removal; 2) Please include general information on how the Horse Pen Creek channel will be restored and stabilized following culvert removal, as well as sequencing information with bridge construction; 2) Note that mechanized clearing impacts will be considered permanent impacts (though not a loss of waters) unless a wetland restoration plan is included and approved; 3) Note that the bank stabilization at Stream 6 will be considered permanent impacts (though not a loss of waters) since the rip rap will not be removed; 4) Please include information regarding the small whorled pogonia survey. Who performed the survey, what was the survey area, when, and what were the results? 5) Please include a cross section of the outfall and rip rap pad of the Stream 6 culvert extension.
