Source of Discharge The Area of Wetness (AOW) identified as "S-4" lies downstream of the toe of an embankment. The embankment was constructed in 2 phases in the 1970s and 80s to enable dry placement of ash materials that had been removed from the 1-4 ash basin. Location of the AOW aligns with a 24" reinforced concrete pipe (RCP) shown on historical drawings. The 24-inch RCP was installed prior to placing the embankment and dry ash to provide an outlet for a natural drainage path and likely to reduce the potential of hydrostatic pressure buildup within the embankment. Plans do not indicate elevation of the upstream end of the pipe or any kind of spring box or aggregate filter but the elevation can be estimated based on terrain grade before ash was placed. Engineering Studies Completed To-Date Investigation of this area has been conducted. The investigation included a geophysical survey, geotechnical borings, piezometer installation and cone penetrometer testing (CPT), along with lab testing to produce a stability analysis of the embankment as well as hand dug test pits and machine dug excavations to locate the pipe. Page 2 of 3 July 22, 2016 Letter to Mr. Landon Davidson The piezometer installed in the embankment crest indicates a steady phreatic surface The analysis of the embankment with actual phreatic surface data yielded an appropriate factor of safety analysis, greater than 1.50. The drawings indicate a weighted reverse filter had been installed over the outlet of the pipe during the 2nd phase of original construction. The presence of the 24-inch RCP pipe was confirmed, with a portion of the pipe uncovered near the toe of the embankment. Water was observed flowing from a joint gap exposed by the excavation and collected in an open pit. Water was pumped out of the pit using vacuum trucks to estimate flow rate. Water continued to flow steadily after pumping for several hours. The flow from the joint gap was higher during the pumping period as water collected inside the pipe was relieved thru the joint gap. The AOW was noted to diminish during this time. After evaluating this flow over an 8 hour period the pipe was sealed back up. No solid materials were observed to be in suspension or accumulated in the pit from this flow. Based on the steady measured phreatic surface, mentioned testing and the observation of water flow it is concluded that the pipe is functioning as intended to keep water pressure from building up behind the embankment and the embankment is stable. This pipe is not "pressurized" other than the developed phreatic head that exists and no body of water exists on the upstream portion of the embankment. Historic Water Quality Monitoring Results Monitoring of S-4 has been conducted on the following dates: a. July 7, 2014 b. May 5, 2015 c. June 29, 2015 d. September 17, 2015 e. November 17, 2015 f. April 18, 2016 g. April 27, 2016 These monitoring results are attached. The July 7, 2014 monitoring results were part of the NPDES permit renewal application submitted on July 28, 2014. That permit issuance is still pending. NC Coal Ash Management Act Compliance Status and Recommendations for Addressing S-4 Per GS 130A-309.210 (d) on September 30, 2014 Duke Energy submitted the proposed plan for the identification of new discharges for the NCDEQ review and approval. Per GS 130A-309.210 (b) on December 30, 2014 Duke Energy submitted the discharge assessment plan to NCDEQ for its review and approval. Per GS 130A- Page 3 of 3 July 22, 2016 Letter to Mr. Landon Davidson 309.210 (c) (2) a. 2. the application of July 28, 2014 identified S-4. Subsequent monitoring analytical results have also been provided to NCDEQ for S-4. Duke Energy is developing a plan for a stone overlay to further reinforce the weighted reverse filter as an interim measure. Presently this area is included in a monthly visual inspection in which results have not noticed a change in the discharge. Duke Energy will increase this inspection frequency to weekly going forward. On April 13, 2016, Duke Energy requested to NCDEQ that S-4 along with a number of other AOW's at various sites, be designated as an effluent channel to the Broad River. This determination is pending NCDEQ review and the issuance of the NPDES permit. With the very low flow and observed concentrations from S-4 water quality standards in the receiving stream are not expected to be contravened. Results of historical ambient monitoring of the Broad River is also attached. This ambient monitoring includes samples collected upstream and downstream of the station and the results are in compliance with the Water Quality Standards.
