Dear Mr. Peterson:
The North Carolina Division of Water Resources conducted, a follow-up inspection of the DuraFiber Technologies
Inc' waste water treatment plant on November 8, 2016 in response to permit violations found during a Compliance
Evaluation Inspection (CEI)conducted on August 4, 2016. This inspection was conducted to verify that corrective
actions have been taken and the facility is operating in compliance with the conditions and -limitations specified in
NPDES WW Permit No. NC0004944.
The _Compliance Evaluation Inspection was conducted by'Division of Water Resources staff from the Mooresville
Regional Office. The following corrective actions to previous violation(s) are noted as follows:
Compliance Issue(s): RAS pumps not in operation, accumulation of sludge and vegetation in effluent
trough on secondary clarifier -
Observation: RAS pump has been repaired and secondary clarifier, with vegetation in effluent trough, -has been
removed from service for maintenance.
Flow meter did not have annual calibration records -
Observation: Flow Meter has been calibrated and calibration date'is being recorded.
Facility did not have sludge processing or sludge disposal records —
Observation: -Past records of,sludge disposal.have been found and future volumes of sludge dewatered are to be
recorded.

Thermometers in Laboratory have not been.checked against NIST traceable thermometer. Laboratory
is certified under' Laboratory Certification Number 200 =
Thermometers in current use have been calibrated against a National Institute of Standards & Technology (NIST)
traceable -thermometer.
It has been observed that remedial actions have been taken to correct problem noted in the initial compliance -
inspection. At this time the Division of Water Resources will not pursue enforcement action for the violation
(NOV-20165-PC-0398), issued on August 11, 2016, provided that facility continues in compliance with permit
conditions.
