Dear Mr. Pittman:
On June 3, 2015, staff of the NC Division of Water Resources (DWR), Water Quality Regional Operations Section,
inspected the subject wastewater system. The assistance of Juanita James was appreciated during the inspection.
The following items specify the permit conditions that were in violation during the time of the inspection.
Effluent Limitations and Monitorine Requirements:
Section A. (1.)states in part,"During the period beginning on the effective date of the permit and lasting until
expiration,the permittee is authorized to discharge treated domestic wastewater from outfall 001. Such discharges
shall be limited and monitored by the Permittee as specified below:
Effluent Characteristics: pH Limits: 6.0-9.0 Standard Units
During the inspection,a violation of this condition was noted as follows:
Effluent pH measured at 5.91 Standard Units

PROPER OPERATION AND MAINTENANCE
Section C.2. states, "The Permittee shall at all times properly operate and maintain all facilities and systems of
treatment and control (and related appurtenances) which are installed or used by the Permittee to achieve
compliance with the conditions of this permit.
During the inspection, a violation of this condition was noted as follows:
The discharge pipe was not located. The discharge pipe was not located following the spill on May 15, 2015
although a dye test was conducted by the maintenance crew from James&James Environmental Management.
To prevent further action,carefully review these violations and deficiencies and respond in writing to this office
within thirty(30)working days of receipt of this letter.You should address the causes of noncompliance and all actions
taken to prevent the recurrence of similar situations. Please also clarify the eluent sample collection point as staff was
unable to collect samples at the collection point indicated by Ms.James.

The permit was renewed and became effective August 1, 2014.
Special Condition A.(2.): Electronic Reporting of Discharge Monitoring Reports(eDMR) is required by
May 1,2015. The May DMR was submitted as required.
Influent pump station contains 2 grinders. Solids are pumped from the pump station by Mike's Septic.
The operation/maintenance logs appreared to be complete.
The flow meter(ISCO 3010 Ultrasonic)was calibrated on 10/24/14 by John Lassiter of Laboratoy
Instrument Services.
Samples were taken during the inspection; all samples were grabs.The results are as follows:
pH =5.91 at 21.3 degrees c (Permit limit is 6-9 Standard Units)-see comments under Operation&
Maintenance section. '

This inspection was prompted by a complaint received by the Asheville Regional Office on 5/15/15. A
spill occurred at the manhole between the VWVfP and the discharge point. There was a blockage in
the discharge line and approximately 100 gallons of wastewater was discharged to the ground. Mike's
Septic pumped solids/Wastewater from the manhole and the blockage(metal cup/bowl)was removed.
A crew from James&James did a dye test to determine the location of the discharge point but the
discharge pipe was not located. The discharge pipe was still not located by the time of the inspection
on June 3rd.
Non-compliance issues:
Operation/Maintenance-discharge pipe has not been located.

Comment: The TRC could not be determined due to the location of the sample collected. A grab
sample was collected at the pipe where the dechlorination tablets are housed,therefore the
sample was not dechlorinated. The reading on the HACH DR 2400 was"over range".
According to Ms. James,the sample collection conditions were not typical on the day of the
inspections flows were low. Also,the samples are normally collected after dechlorination.
We could not physically obtain a sample from the normal post-dechlorination collection
point'
