Rockingham County Dear Mri Rowell: i Ii accordance with your request for permit modification received January 17, 1989, wb are forwarding herewith modifications to the subject permit. Your request for str"m monitoring locations on the Smith River prior to its confluence with the Dan River and on the Dan River at the Highway 87 Bridge has been evaluated but -will not be included within the conditions. However, the upstream monitoring location has been changed as follows: Upstream So feet above the discharge. The downstream sampling station will remain at N. C. Highway 700 as stated in the permit. In; addition, BADS samples may be taken as grab instead of composite samples at the upstream and downstream locations as specified in the permit. However, the annual priority pollutant analysis requirement must remain in the permit since it is a requirement for all major facilities and primary industries. Attached is a revised effluent sheet which should be inserted into your permit.; The old effluent sheet should be discarded. A1111l'other terms and conditions contained in the original permit remain unchanged and in full effect. These modifications are issued pursuant to the requirements of North Carolina Genexal Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency dated Doeambsr 6, 2989. This permit modification becomes effective immediately. If this decision is unacceptable to you, you have the right to an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150D of North 0aroline General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604.
