Dear Mr. Adams:
Ron Boone and Min Xiao of the Winston Salem Regional Office (WSRO) of the North Carolina Division of
Water Resources (DWR or the Division) conducted a compliance evaluation inspection of the East Bend Industrial
Park Wastewater Treatment Plant on January 12, 2017. The assistance and cooperation of Mark Matthews, Operator
in Responsible Charge (ORC), was greatly appreciated. An inspection report is attached for your records and the
inspection findings are summarized below.
The East Bend Industrial Park Wastewater Treatment Plant is located at the south end of Salem Trail, in
East Bend, Yadkin County, North Carolina, at approximate coordinates 36.2113360, -80.494283°. The Town of East
Bend is authorized to operate this 0.01 million -gallon -per -day (MGD) wastewater treatment plant, which consists of
extended aeration, settling, sludge holding, UV disinfection, and backup chlorination and dechlorination, and discharge
treated effluent from outfall 001 of said treatment works into an unnamed tributary to the Yadkin River, this section of
which is currently classified as Class WS -IV waters and is located in the Yadkin Pee -Dee River Basin.
SITE REVIEW
Mr. Boone and Ms. Xiao reviewed the entire plant with Mr. Matthews. Everything was in good condition and
well maintained. No discrepancies or violations were noted.
DOCUMENTATION REVIEW
Mr. Matthews had all required, paperwork on hand. This includes discharge monitoring reports, laboratory
reports, chains of custody, operation and maintenance logs, operator visitation logs, etc. The only discrepancy Mr.
Boone and Ms. Xiao noted was that the Town does not currently have a properly designated certified backup operator
(BORC). This must be addressed immediately. The former BORC resigned his position on June 30, 2016, so the plant
hasn't had a BORC for six months now. By regulation, a permittee has 120 days from the date a position is vacated to
designate a new ORC/BORC. A BORC with at least a grade 1 biological wastewater certificate must be designated as
soon as possible. You may initiate this process by filling out and filing an official ORC/BORC designation form, which

Last year's inspection revealed that the town did not have a proper field lab certification. This was addressed
and the town does now have the proper certification credentials.
There were no other discrepancies noted. Mr. Matthews is doing a good job operating and maintaining the
plant, but should be slightly more attentive to the record keeping and documentation requirements of the permit, as
illustrated by the current lack of a BORC, as well as last year's lack of a proper field laboratory certification. Please
remember that violations of the permit are subject to enforcement actions not to exceed $25,000 per day, per
violation.
