WILLIS RECEIVEDINCDENWR JAN 2 3 2017 Water Quality Permitting Section The City of Lenoir has been under the above referenced AOC from the EPA for the Gunpowder Creek Wastewater Treatment Plant. The City has submitted all quarterly reports and has maintained communication with EPA regarding the project completion and start-up in accordance with the requirements of section ]111. 13-16 of the AOC. All work has been completed as required and in accordance with the compliance schedule. The City of Lenoir respectfully requests a determination by the EPA that the City. of Lenoir has fully completed and implemented the actions required by this Administrative Order on Consent. Atlanta, Georgia 30303-8960 Subject: Fourth Quarter 2016 Report for City of Lenoir, Gunpowder Creek WWTP (NPDES Permit # NC0023736); CWA-04-2013-4775 Dear Ms. Jones: This is the fourth quarter 2016 report required by the Administrative Compliance Order on Consent #CWA-04-2013-4755, 111. 13 — 16. Fourth Quarter 2016: October — December 2016: Permit Effluent Limit Exceedances - There were no violations of the permit limits / SOC limits during the period for the Gunpowder Creek WWTP. Actions Taken and Progress Made in Achieving Goals of Engineering Plan Construction at the treatment plant was completed during the third quarter of 2016 as reported in the last quarterly update. The plant continues to operate well with effluent limits being consistently met. Other Actions — There are no other actions to report. I would like to inform you that several staffing changes have been made in the Treatment Plant Operations Department. Lisa Triplett was promoted to Treatment Plants Superintendent and Donnie Hawkins was promoted to Asst. Superintendent and ORC. They have a combined 50 years of experience working with our treatment plant facilities. The City of Lenoir has submitted all quarterly reports and has maintained communication with EPA regarding project completion and start-up and the replacement of the chlorine feed system which was required by June 21, 2016. All work has been completed prior to the compliance dates. The City of Lenoir respectfully requests a determination by the EPA that the City of Lenoir has fully completed and implemented the actions required by this Administrative Order on Consent. Certification Statement - I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance to a system designed to assure that qualified personnel properly gather and evaluate the information submitted.
