I have included the Attachments G& L with a project description and a set of the project plans. A stormwater management plan is also attached. The existing 160' long X 22' wide, reinforced concrete deck on steel I-beams, supported by reinforced concrete caps and timber piles with reinforced concrete posts and beams, four span bridge is structurally deficient and will be replaced with a new, triple span 160' L X 30' W, cored slab, box beam bridge at the same �Nothing Compares�,�_ Sta�e ofNonh Carolina � Depanment ofTlansponation � Division ofHighways, Division 7 OtTice 1584 Yanceyville Street, Greensboro, NC 274051 p� �� Box 14996, Greensboro, NC 27415-4996 336-087-0000 location. An off-site detour will be utilized during construction. The project only proposes 35 LF of temporary stream impacts associated with a temporaiy causeway to remove the existing concrete bent in the stream. Deck drains are installed only above the floodplain areas to promote infiltration prior to stormwater reaching the surface stream; there are no deck drains directly over the channel. Stormwater runoff will also be captured at a proposed drop inlet and discharged to a rip rap pad on the south west side of the bridge and outlets at non-erosive velocities. Exisring roadside ditches will be retained and lined with toe protecUon using PSRM along the fill slopes. These ditches outlet in wooded areas in excess of 100 feet from the stream and therefore becomes diffuse flow before entering the channel. 30aks personnel surveyed Jacobs Creek on August 30, 2016 for the James Spinymussel (Pleurobema collina) and the Roanoke Logperch (Percina rex). No specimens of either species were found and NHP has no record of them occuiring within 1 mile of the project site. A"no effect" determination was made and concurred upon by Mr. Gary Jordan (USFWS) for the mussel; his biological conclusion for the logperch was "may affect, not likely to adversely affecY' and is documented in the attached letter. Robert Lepsic conducted the habitat suiveys for the Smooth Coneflower (Echinacea laevigata) on 7une 28, 2016. He determined that there is habitat for the named species within the project area, however, no species were found. Therefore, the biological conclusion is "No effect, Habitat presenY'. The Bald Eagle is protected in every county in North Carolina under the Bald and Gold Eagle Protection Act; however, no suitable feeding/water source is located within one mile of the project study area to suppart Bald Eagle habitat. Also, Mr. Gary Jordan (USFWS) recently released the Programmatic Conference Opinion (PCO) for the recently listed Northern Long Eared Bat (NLEB). This opinion, states in part, that ".., it is the Service's conference opinion that NCDOT activities in eastern North Carolina (Divisions I-8), as proposed, are not lilcely to jeapardize the conttnued existence of the NLEB.
