Please find attached the certificate of completion notices for the Cedar Lakes project in Rolesville
(SAW-2004-20555 & DWR 14-0874).

You’ll notice that the bottomless culvert / arch span was changed. The plans included in the PCN
had a 25’ wide x 10’ high span with a bend/miter in it. What they actually installed was a 30’ wide x
18’ high arch span with no bend/miter. It was also slightly shorter in length, then the one included in
the application.

Please let me know if you need anything else or would like to visit the site.

PS … Debbie was the PM on this one and handled the permitting, but was unable to visit the property
post-construction, so I filled in for her, which is why I’m submitting these notices.
