No forms needed. Just include a cover letter requesting a minor modification of the existing Major Permit, the
purpose and need of the proposal, and describe what DOT proposes to do. Also please include a plan view and

x-section of the riprap, as well as a WBS # and authorization to debit the $100 application fee. Just let me know
if I can be of any further assistance.

I am working on the permitting for Sunset Beach Causeway rip rap slope protection. I believe Mason spoke with you
about this and it was going to be a minor mod to the existing permit. Please let me know what you will need as far as
applications filled out for this request. Also, you will need a planview and a cross-section anything else?
