Subject: SR 1513A (Indian Grave) - Sediment Loss

Heath,

I wanted to notify you about sediment loss that occurred on SR 1513A (Indian Grave). The project received almost two inches of rain on 11-29 and 11-30. The contractor began EC repairs
late on 11-30 along the
roadway. The contractor was unable to move equipment down the slopes until 12-2 to begin reclaiming the sediment loss, due to slick conditions. The contractor is actively removing the
lost material and is restoring the
erosion control measures. Below are the locations and the actions being taken.

42+11 (permitted) – approximately a wheelbarrow full of material at the silt fence outlet. The basins are being repaired.

63+50 (permitted) – approximately an excavator bucket of material The basins are being repaired. In addition, we are adding an additional basin between the two shown on the plan. Also,
we will be adding a
temporary berm along the top of the fill slope with a slope drain to divert additional runoff into the existing basins.

68+50 – approximately one five gallon bucket of material lost underneath the silt fence at pipe inlet. Sediment is being removed along the toe of the silt fence, a basin and two SSCF’s
are being added to address
the buildup.
